Job Title -Case Worker Pay Rate- $28-$30/hr Job Type - Full Time Location - Northport NY 11768 Work Schedule: Standard hours are Monday–Friday, 9am–5pm; however, the facility is staffed around the clock, allowing for flexible scheduling. Requirements • MSW (Masters Degree in Social Work) Required; LMSW (Licensed Master of Social Work) or LMHC (Licensed Mental Health Counselor) preferred, • Prior case management experience preferred, • Experience with HMIS or similar data entry platforms preferred, • Experience in residential programs serving individuals experiencing homelessness or similar populations preferred, • Complete initial needs assessments, respond to critical needs, and request Military Service Records (DD214) during intake, • Maintain confidentiality, adhere to ethical standards, and orient clients to program rules and expectations, • Ensure clients' basic hygiene and nutritional needs are met, • Develop and monitor Independent Living Plans (ILPs), track weekly progress, evaluate outcomes, and refer clients to appropriate resources, • Maintain complete and accurate case records, including ILPs, progress notes, correspondence, documentation of violations, and updated HMIS entries, • Report critical incidents immediately to the Program Director and provide regular updates on client progress, including behavioral or appearance changes, • Collect data required for funding and compliance, including Contract documentation, • Participate in fire/emergency drills and demonstrate the ability to properly use emergency panic buttons, • Follow up with maintenance staff regarding repairs or replacements, • Ensure clients who were inappropriately referred are relocated within 48 hours or the next business day, • Collaborate with the VA liaison to help clients meet their goals, • Conduct oneself professionally and with accountability to the agency, community, and client population, • Maintain reliable attendance, avoid misuse of sick/personal time, arrive on time, and provide proper notice when late or absent, • Perform on-call duties according to policy and be available for overtime when needed, • Prioritize workflow effectively, work independently with minimal supervision, and maintain confidentiality of all client and program information, • Report to work well-groomed, follow the dress code, and wear an ID badge while on duty, • Interact positively and professionally with clients, visitors, contractors, volunteers, and staff, • Contribute to a supportive, positive work environment and project a professional image to the public, • Demonstrate willingness to go above and beyond to help maintain a high-quality, caring shelter environment, • Perform other duties as assigned Benefits Summary: • Paid Time Off (PTO), • Voluntary Benefits: Medical, Dental, Vision, Life Insurance, 403(b), LegalShield, and Aflac, • Employer-Paid Benefits: $15,000 Life Insurance and $15,000 AD&D, • Dependents & Domestic Partners: Eligible for coverage under all available plan
