This is a true case management role — not high-volume visiting. What the role looks like: ~4 visits/day (manageable, not overloaded) Clustered Queens territory (minimal travel) Mix of admissions, recerts, follow-ups Strong care coordination with MDs, social work, interdisciplinary team Light on-call (triaged, filtered) No chaotic scheduling — structured days Schedule: Monday–Friday Day shift No weekends Ideal background: RN / Registered Nurse Hospice RN, Home Health RN, or Case Manager RN Experience with OASIS, admissions, recerts preferred Bilingual Spanish or Cantonese required Why this role stands out: Most hospice roles in NYC are high volume and burnout-heavy — this is not that.
