Job Description The RN (Registered Nurse) Case Manager is responsible for providing Rebekah Certified Home Health Agency patients with quality care in a safe and judicious manner. The Case Manager will assist in facilitating and coordinating all services that the Home Health Agency patients require. He or she will be responsible for promoting quality, patient access care, cost-effectiveness within a well integrated healthcare delivery system. In addition the nurse will be responsible for the on-going assessment, treatment and evaluation of the patient’s condition and family situation, as well as the supervision and evaluation of aide personnel that are assigned to provide services to the Home Health Agency patients. About The Role: We are seeking a dynamic and experienced Case Manager to support individuals with chronic medical conditions, including HIV and AIDS, in the community. Provides person centered support and helps the individual to build on his or her strengths in life activities.Assists the individual meet his or her physical and personal care needs (i.e. toileting, bathing, grooming) and provides training in these areas when needed. Drive program vehicles in safe and responsible manner and document all vehicle operations in travel logs.Ensure adequate supervision and that all safeguards and plans are followed; in addition to monitor the interaction, activities and response to services. Ensure to follow mealtime safeguards to include but not limited to; prescribed diets, meals plan and choking preventions.Utilizes effective professional and empathetic interpersonal skills when counseling, supporting or negotiating to handle sensitive issues with families and outside agencies.
