Job Description JOB DESCRIPTION JOB TITLE: Case Manager-NYSSHP Singles/Yonkers RA Program DEPARTMENT: Adult Housing & Services Department FLSA: Non-Exempt REPORTS TO: Program Director LOCATION: Yonkers, NY SALARY: $40,000.00 to $45,000.00 SUMMARY: The Housing Retention Case Manager provides, home-based services and support to disable, non-disable, employed and employable single adults in permanent housing. Participants are provided with a variety of services to promote housing stability and individual well-being. Housing Retention Case Manger is responsible for following up with Coordinated Entry referrals, for completing the lease packets, ensure the administrative compliance and collection of rent, delivery of rent, for site owned or managed by Westhab. The Housing Retention Case Manager must diligently work to protect and preserve the quality of life in, and the value of, the properties which they are assigned to manage. This position requires a 24/7 work phone availability in case of any emergency. DUTIES & RESPONSIBILITIES: • Responsible for conducting clients’ intakes and obtain needed documentation to ensure clients eligibility for the referred department/program. Maintains monthly and accurate and up today progress notes in HMIS as directed., • Able to travel within the Westchester County and attend DSS/SSI/SSD or medical appointment for the clients if needed., • Able to work one or two weekend day in the year as requested by Director, if needed. Able to work some evenings during the month if needed and as requested by Director, • Works closely with the Department of Social Services and the Westhab Housing Department staff to achieve client’s permanent housing goals as well as with shelter and transitional shelter staff., • Works closely with Westhab various department to ensure compliance, and program safety; Compliance Department, Security Department and the Facility Maintenance Department., • Attends weekly cross departments meetings and others as required. Create and maintain ongoing communication, both verbal and written, between all parties involved (Shelters, DSS, and collaborating agencies, etc.), • Identify client’s barriers and develops a service plan with clients to address the identify barriers. Responsible for referring clients to mental health and substance abuse services if needed., • Promotes housing stability and individual well-being during program enrollment. Conducts monthly room inspections and building inspections., • Responsible for collecting and submitting all of the initial and or lease documents as directed by program director., • Responsible for reports, assessments, HQS Inspection, HUD required documentation completion and rent reasonableness in a timely manner. Responsible for keeping the client’s folders up today., • Requires the completion of various programmatic time sheets and activity log along with maintaining Google calendar updated., • Ensures that clients are following the program rules and regulations. Responsible for conducting daily wellness check with the most vulnerable clients. Serves as mediator for the clients when needed. Knowledge of crisis interventions technics., • Responsible for scheduling monthly client’s meetings in person to sign needed documentation or as often as needed. Identify program needs and makes recommendations. Bachelor Degree or Associate Degree required in social work or related field preferred, with two years’ experience in direct client services. Fluency in Spanish is highly desirable. Detail oriented to complete requirements of files and contract compliance packets. Excellent written and communication skills. Possess strong organizational skills with the ability to meet a demanding workload. Knowledge of community services, conflict and resolution, and strengths based case management. Some evenings and weekends as needed. Must have own transportation and valid driver’s license.
