SUMMARY Under general direction, is responsible for designing and implementing an organized, individualized program for acquisition of functional skills that will lead to increased independent community participation for the individuals served. This is done in accordance with regulations and requirements of the agency and applicable governmental and other regulatory agencies. This is a management position that is responsible for providing supervision to the Habilitation Assistants. ESSENTIAL DUTIES AND RESPONSIBILITIES The duties and responsibilities of the Habilitation Specialist will include, but are not limited to the following: • Serves as a liaison between agency, the people we support, and families. Serves as advocate and provider for all issues pertaining to those receiving services., • Develops programs and day habilitation plans for persons served., • Implements active treatment plans for program participants, both inside and outside of building., • Supervises non-professional staff in implementation of day habilitation plans., • Participates in individual program planning process., • Orients, trains, evaluates, delegates, supervises work of, and recommends hiring and disciplining of subordinate staff., • Recognizes and observes important relationships with Service Coordinators for participants’ program development and referral for other kinds of assistance as needed., • Maintains attendance and other required records/paperwork., • Utilizes community resources, as appropriate for implementation of day habilitation plans and community integration., • Carries out basic administrative tasks related to caseload and other selected assignments as directed by program director or program coordinator., • Plans group and individual activities and assigns staff to implement them., • Trains and instructs support staff in daily routine, program planning, and general procedures and practices of the group., • Attends regularly scheduled supervisory staff meetings., • Prepares and adapts materials and plans and sets up furniture and equipment for program activities., • Participates in scheduled in-service training programs., • Provides support/coverage to other classrooms as needed., • Participates in activities outside of activity room when necessary, i.e. trips, pool, gym. Enters swimming pool with the people we support as required., • Selects and utilizes educational materials., • Teaches persons served to become more independent., • Drives agency vehicles for individual transportation, pickups, deliveries, community outings, and medical emergencies as required., • Assists persons served with transportation between buses and buildings as needed., • Performs direct care responsibilities that include feeding and toileting, as needed., • Initiates precautions within established framework to assure that own work is completed in a manner free from recognized hazards., • Performs other related duties as requested. The program is designed to provide long-term residential care and twenty-four-hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being. The Part-Time Residence Manager is responsible for the supervision of the residents and the overall operation of the facility during designated shifts. ESSENTIAL DUTIES AND RESPONSIBILITIES The duties and responsibilities of the Residence Program Specialist will include but are not limited to the following: • Provides care and assists with personal development and independence of the people we support., • Ensures that all measures for safety and well-being are maintained at highest level., • Provides training for the people we support in activities of daily living and in development of self-help and social skills., • Maintains a personalized, homelike atmosphere in the residential facility., • Attends weekly staff meetings as well as any other assigned meetings and in-service trainings., • Prepares concise written reports including Incident Reports, Progress Notes and other reports as assigned., • Responds to crisis situations and medical emergencies, including accidents by taking quick and decisive action., • Ensures actions are conducted in a manner that prevents abuse, and reports observations of co-workers who may be abusive in accordance with established procedures., • Administers appropriate first aid based on visible symptoms and medical history of the people we support in accordance with established guidelines and accepted first aid practices., • Summons police, fire, ambulance or other emergency personnel if situation warrants it., • If an emergency has caused a person we support to exhibit behavior that is threatening or dangerous, may have to utilize strategies for crisis intervention in accordance with OPWDD approved Behavior Management guidelines., • Provides transportation for the people we support in agency vehicles to and from work, shopping, programs, restaurants, movies, doctor’s appointments, etc., • Assists with meal planning and preparation as well as with purchasing of food and supplies., • Demonstrates an understanding of and implements behavioral programs., • Participates fully as a member of interdisciplinary plan., • Maintains an ongoing informal exchange of information.
