SUMMARY The Vice President of Clinical Services provides executive leadership, strategic direction, and operational oversight for ADAPT Community Network’s clinical service delivery system, including all Article 28 Clinics and the organization’s medical affiliate, ADAPT Medical Professional Services (MPS). This role is responsible for ensuring the delivery of high-quality, accessible, person-centered healthcare services to individuals with disabilities across a broad spectrum of medical, rehabilitation, and mental health services. Job Description The RN (Registered Nurse) Case Manager is responsible for providing Rebekah Certified Home Health Agency patients with quality care in a safe and judicious manner. The Case Manager will assist in facilitating and coordinating all services that the Home Health Agency patients require.
