We have cases available throughout Brooklyn, Queens, Manhattan, the Bronx, and Staten Island, with assignments based on your preferred location, ZIP code, and availability. Responsibilities Provide one-on-one speech therapy services to children and adults in their homes. Conduct comprehensive evaluations of speech, language, communication, cognition, voice, fluency, and swallowing abilities, as appropriate. Develop, implement, and update individualized, goal-oriented treatment plans in accordance with physician orders and agency requirements. Provide evidence-based, patient-centered treatment to improve functional communication and/or swallowing abilities. Educate patients, families, and caregivers regarding communication strategies, home programs, swallowing precautions, and carryover techniques. Collaborate with families, caregivers, physicians, and interdisciplinary team members to ensure continuity of care. Monitor patient progress and modify treatment plans as clinically appropriate. Maintain accurate, timely, and compliant clinical documentation. Participate in interdisciplinary meetings and care coordination as needed. Adhere to all professional, ethical, and organizational standards of practice. Qualifications Current New York State Speech-Language Pathologist license. Experience working with pediatric and/or adult populations. Experience working with individuals with Intellectual and Developmental Disabilities (ID/DD) is preferred. Home care and/or community-based experience is a plus. Experience with speech, language, cognitive-communication, and/or swallowing disorders is preferred. Strong clinical assessment, documentation, and communication skills. Compensation $65–$70 per 30-minute treatment session. MINIMUM QUALIFICATIONS Candidates must meet applicable New York State Early Intervention Program requirements for Service Coordinators and Angel Community Alliance credentialing requirements.
