Department/Unit: HBD - Neurology Work Shift: Day (United States of America) Salary Range: $83,200.00 - $128,960.00 The Registered Professional Nurse in the ambulatory setting provides care across the life span to individuals, families, caregivers, groups, populations, and communities. This is accomplished during face-to-face encounters or through a variety of telecommunication strategies, often resulting in the establishment of long-term patient care relationships. About the Role: The Senior Manager, Demand Planning has responsibility for creating and maintaining a statistical demand forecast, effectively fitting and tuning statistical models to project seasonality and market trends, actively participate in the S&OP process, and ensure data insights and demand signals are provided to the business. Oversees statistical reporting., • Ensure service goals and objectives are being properly implemented., • Conduct internal chart reviews in collaboration with the QA department to ensure high standards of care and high-quality service delivery., • Conduct individual and group supervisory meetings based on staff and team needs. This includes bi-weekly staff meetings., • Interview for staff and other positions as needed, coordinating selection and new hire process with Human Resources Department., • Appropriately respond to/report needs as identified by tenants, family members, staff, regulatory agencies, etc., • Promote strategies that support learning and professional development, including the creation and maintaining of a monthly training schedule for Supportive Housing staff in Rockland/Westchester., • Assist staff with correspondence/communication with family members, health care professionals, other providers, etc. Support/interact with families as needed., • Provide clinical supervision in the development of service plans and in assessing for safety planning with staff., • Work collaboratively with internal interdisciplinary teams and community partners to enhance tenant well-being as well as identify community resources, facilitate referrals, and serve as a liaison with various care coordination agencies, health and mental health providers, substance use programs and employment providers to secure services. This includes SPOA meetings in Putnam, Rockland and Westchester., • Develop supervision plans and accountability systems that manage reporting requirements and service standards., • Ensure staff utilize a harm reduction approach to support tenants around issues of substance use. This includes reports such as High Risk List, Admin letters, Accept/Denial documents, Admittance/Discharge/Hospital records, LPHA requests and Healthe Connections reports., • Collaborate with staff in assisting tenants with meeting their financial needs program fee obligations., • Participate in after-hours coverage and respond to needs of tenants/staff in all programs when on-call. Update supervisors of all on-call issues., • Perform additional duties as per the nature of the position or as requested by Sr. Director/Chief Program Officer. Requirements: • Bachelor’s or Master’s degree in Social Work, Psychology, Counseling, or a related field., • Relevant experience in Supportive Housing or a related supervisory role., • Knowledge of Supportive Housing guidance, principles, and best practices., • Strong leadership and team management skills., • Excellent interpersonal and communication skills., • Ability to work collaboratively within a team and engage with the community., • Commitment to maintaining confidentiality and professionalism., • Proficiency in Microsoft Office suite and database management systems., • Fluency in English (Reading, Writing, Speaking)., • Must be cleared and maintain clearance by applicable regulatory clearances as required., • Legally eligible to work in the United States without sponsorship. Special Considerations: This is a full-time, exempt position with a regular daytime schedule that may include an evening shift and rotating on-call responsibilities with the Sr. Director of Supportive Housing and the Supervisor. Hours per week, reporting structure and location are subject to change per program needs. This position may require availability outside of the standard workday, and as such must be ready and available at times to direct and support staff either by phone or in person if needed during the evenings, weekends and holidays.
