EDUCATIONAL REQUIREMENTS: Bachelor's Degree or Master's Degree in one of the following fields preferred: Social Work, Psychology, Education, Rehabilitation, Occupational Therapy, Counseling, Community Mental Health, Sociology, Speech and Hearing, Physical or Recreational therapy. Degrees in other related areas may be considered. EXPERIENCE REQUIRED: • For H.S. Job Description JOB DEFINITION: The Psychiatric Rehabilitation Care Coordinator (PRCC) functions as a member of a multidisciplinary treatment team to provide skill-building, vocational, and care-coordination services to individuals diagnosed with mental health and/ or substance use disorders. The PRCC partners with clients to identify social, community, and skills-based needs, and works with individuals to develop skills needed for meeting those needs. This role directly serves clients, engages with clinical treatment team members, and advocates with community-service agencies to reduce reliance on the behavioral health system, increase wellness self-management, and promote recovery. EDUCATIONAL REQUIREMENTS: Bachelor's Degree in one of the following fields: Social Work, Psychology, Education, Rehabilitation, Occupational Therapy, Counseling, Community Mental Health, Sociology, Speech and Hearing, Physical or Recreational therapy.
