This includes; providing excellent customer/patient services via phone and in person; referring callers to the appropriate departments; making appointments for patients according to established center protocols; assisting patients/clients with forms; pre-registration and eligibility/insurance verification. The position is considered a crucial link between the patient and the care delivered by the clinical and service staff of the center. The incumbent must work collaboratively with all clinical services staff in support of direct patient care while demonstrating excellent customer service. Patient services are a core role in the health center that requires the receptionist to serve as a point of contact with other internal and external departments, all with the goal of fostering an environment which promotes patient comfort and trust. Essential Functions: • Welcomes and greets patients/ visitors to the department in a manner that is helpful and friendly; determines purpose of visit and direct patients/visitors to appropriate person or department(s)., • Check’s in and registers patients upon arrival., • Assists in coordination of patient flow to allow the clinic to serve an adequate number of patients as established by grant requirements., • When scheduling appointment(s), carefully screens patients for required demographics, and can identifies new and established patient visits based on the Clinica Sierra Vista scheduling policy., • Informs patient of any existing balance noted in computer during registration and educate on future balances and financial responsibility for services rendered., • Provides information to Clinic callers according to the Clinic’s Policies and Procedures; transfers calls as needed, • Must exercise utmost diplomacy and tact to provide excellent customer service for patients; practices confidentiality and privacy protocols in accordance to Clinic policies and HIPAA requirements., • Verifies method of payment for service (Medi-Cal, Medicare, private insurance, private pay, Healthy Families, etc.) and collects data and/or co-payment as appropriate., • Completes daily batch reports and cash collections drop following daily operations and ensures reports are forwarded appropriately and timely to accounts receivable., • Exercises problem-solving and conflict resolution skills when handling patient complaints; refers patient complaints to appropriate designated personnel as needed., • Performs day-to-day administrative functions and general office duties including but not limited to word processing, copying, filing, faxing, answering phones and data entry., • Performs all duties in support of successful EHR/EPM utilization., • High School graduate or equivalent., • Must be 18 years of age., • Minimum of two years experience in a medical setting., • Knowledge of community; skilled in communicating with people and understanding their problems., • Should believe in health care with dignity for all., • MA Certificate, • Must adhere to Clinica Sierra Vista’s employee health/immunization requirements or provide a valid exemption request for subsequent approval. This role is responsible for reviewing CPHW documentation, monitoring lab results for abnormalities, coordinating provider orders within the Electronic Health Record (EHR) system, and providing health education to pregnant patients enrolled in Comprehensive Perinatal Services Program (CPSP) services. The RN plays a key role in coordinating care and improving maternal and fetal health outcomes. Essential Functions: • Case coordination of Obstetric patients enrolled in CPSP services as directed by the Women’s Health Director, California Department of Public Health and CPSP protocols., • Educate patients enrolled in CPSP with up- to- date information, providing higher quality of care., • Facilitate implementation of changes in evidence-based care as it relates to Maternal Health., • Review prenatal records for high-risk factors and follow direction of Obstetric Provider to determine level of care appropriate for the patient., • Assess patient Obstetric, Medical and Family history for any concerns that increase risk to pregnancy and forward to Obstetric Provider for review., • Provide patient education regarding access to prenatal care from outside agency if patient’s pregnancy deemed high- risk requiring a higher level of care., • Ensure the Individualized care plan is complete and appropriate for patient needs, assess effectiveness of interventions and modify care plan as needed., • Ensure patient receives integrated care that addresses patient needs and promotes patient involvement., • Assess psychosocial, nutrition and health education need of the patient and ensure appropriate community resources and referrals provided, including but not limited to, housing, transportation, behavioral health, dental, genetic screening, and intimate partner violence., • Assess patient weight gain in pregnancy and refer to nutrition services as needed, • Provide patient education on risk factors for the pregnancy and prevention of complications., • Assess patient understanding. taking into consideration language barrier, education level, presence of any disabilities, cultural and gender diversity., • Collaborate with Obstetric Provider regarding patient care plan., • Function as the liaison between the patient and the health care team to promote effective communication., • Educate patients on the purpose for testing, labs and interventions, • AA/AS in a health, social science or related field required and/or graduation from an accredited School or License in Registered Nursing. Job Description Job Summary: The Employment Specialist working under the Homeless Veterans Reintegration Program (HVRP) grant at Vocational Rehabilitation Specialists, Inc. (VRSI) is responsible for assisting homeless veterans in securing and maintaining employment. This role involves providing comprehensive employment services including job readiness training, career counseling, job placement assistance, and ongoing support to ensure long-term job retention. The ideal candidate will be passionate about helping veterans reintegrate into the workforce and improve their quality of life. Company Overview: Vocational Rehabilitation Specialists Inc. (VRSI) is committed to aiding Military Veterans through the Department of Labor's Homeless Veteran Reintegration Program (HVRP). With 11 offices across the United States, VRSI provides vocational counseling, training, vocational evaluation, and direct employment services aimed at helping eligible veterans reach their full vocational potential. Key Responsibilities: Client Assessment and Case Management: • Conduct comprehensive assessments of veterans' skills, experience, barriers to employment, and job readiness., • Develop individualized employment plans that outline veterans' career goals, action steps, and timelines., • Provide training on resume writing, job search strategies, interview preparation, and professional conduct., • Offer one-on-one career counseling to help veterans explore career options and make informed decisions., • Build and maintain relationships with local employers to identify job opportunities suitable for veterans., • Collaborate with employers to understand their hiring needs and promote the benefits of hiring veterans., • Maintain accurate and up-to-date client records, case notes, and employment outcomes., • Work with community partners, veteran services organizations, and other stakeholders to coordinate services and resources for veterans. Education: Associate’s degree in social work, human services, counseling, or a related field. Relevant experience may be considered in lieu of a degree. Experience: Minimum of 2 years of experience in workforce development, employment services, or a related field. Experience working with veterans or homeless populations is highly preferred.
