Position Introduction: Are you a registered nurse with strong communication skills and a passion for patient advocacy? Do you enjoy building relationships while driving clinical impact? If so, we invite you to join our team as a Clinical Transition Coordinator. In this unique role, you’ll combine your clinical expertise with business development by partnering with hospitals, skilled nursing facilities, and physicians to ensure patients receive safe, seamless infusion therapy transitions from hospital to home. You’ll serve as a trusted patient advocate, a connector for healthcare partners, and a champion for our mission to deliver high-quality, compassionate care. What You’ll Do • Build and maintain relationships with physicians, case managers, discharge planners, SNFs, ALFs, and payers., • Generate new referrals by educating referral sources on infusion therapies (TPN, antibiotics, inotropics, nutrition)., • Advocate for patients by coordinating safe and timely care transitions., • Conduct infusion set-ups, provide caregiver/patient education, and validate prescriber orders., • Collaborate with pharmacy, nursing, operations, and reimbursement teams to ensure continuity of care., • Deliver engaging in-services and presentations to referral sources., • Meet and exceed referral, revenue, and territory growth goals., • Have a current NY State RN license as required, • Have a valid driver’s license as required, • Possess excellent communication and presentation skills, • 3+ years of clinical or homecare experience, • Prior sales/marketing experience, • This role offers a predictable schedule with no nights, weekends, or holiday shifts required, giving you the flexibility to enjoy your personal time while building meaningful relationships and making a clinical impact., • Be part of a mission-driven team improving patients’ lives through infusion therapy., • Use your clinical expertise in a dynamic, relationship-focused role., • Enjoy competitive pay, benefits, and opportunities for professional growth.
