Job Description POSITION TITLE: Home Visitor DEPARTMENT: Early Childhood LOCATION(S): The Bronx REPORTS TO: Deputy Director of Homebased/FCC/CC SALARY RANGE: $32,672.64 - $37,804.80 (benefits include life insurance, 401k plan and vacation, personal and sick time-off) DATE: December 2025 SCHEDULE: Monday - Friday, 8am - 4pm, or 9am - 5pm. Schedule may vary due to program needs JOB SUMMARY: Provide high-quality, culturally competent child development and parent support services with an emphasis on the role of the parent as the child's first and most important relationship. The Home Visitor will also assist with the completion of the child health requirements and other required screenings and assessments while supporting the larger system of Family and Health/ Nutrition Services. For the start-up phase for 2 years this will be an evolving position. Interim roles and responsibilities will be adjusted and this position requires an individual to be flexible. Job responsibilities will be revisited once all center-based sites are up and running. EDUCATION, EXPERIENCE AND SKILL REQUIREMENTS: • Minimum - High School Diploma and Home- Based CDA credential or comparable credential, or equivalent coursework as part of an associate’s or bachelor’s degree (60-90 college credits in Social Work, Human Development or Early Childhood Education preferred), • Experience working with young children and understanding of principles of adult learning, • Experience working with and knowledge of community resources, • Ability to effectively communicate orally and in writing with family, staff and community members, • Ability to foster and maintain positive, professional relationships with participating families, • Ability to use public transportation and travel independently, • Must be able to work independently, • Excellent computer skills including strong working knowledge of Microsoft Word and Excel, • Successfully clear; NYS DOI Fingerprint Screening, NYS Central Registry Clearance, Sex Offender Registry Clearance, 3 reference checks, physical examination with updated TB test, Tdap, MMR, varicella (fees may apply), • Meet all employee health requirements in Article 47 (DOHMH) and Head Start performance standards, • Mandated Reporter training certificate (every 2 years) and comply with agency’s policy and procedure regarding identification and reporting of child abuse and neglect, • Obtain First Aid/CPR and Preventive infectious diseases certificate within the first year of employment. Under the direct supervision of the Education and Disabilities Coordinator, the Home Visitor shall be responsible for: • Maintaining a caseload of (12) participants, • Providing 90-minute weekly home visits to support parents/guardians in their roles as primary caregivers and to facilitate the child’s individual development within their home environments, • Providing regularly scheduled home visits with enrolled pregnant women to ensure access to comprehensive prenatal and postpartum care and to provide prenatal education on topics such as fetal development, labor and delivery, postpartum recovery (including maternal depression), and the benefits of breastfeeding., • In collaboration with the Education Coordinator and assigned staff, planning and implementing developmentally appropriate, parent/child focused Group Socializations twice per month, • Assisting parents in identifying strengths and goals leading to school readiness, • Working in collaboration with families to identify goals for themselves, their families and their child, and developing objectives for meeting those goals for the Family Partnership Agreement, • Record Keeping: Enters child and family information into the approved child tracking system database (COPA & ACS/DOE systems). Assist with collecting child health medicals and dentals along with entering health related data into COPA. Submits reports as needed and requested, such as monthly summary tracking reports, etc. Maintains and provides data for the annual Program Information Report (PIR). Records, monitors and follows up on referrals for support services for reporting requirements and ensures that all documentation is current in child’s file and in compliance with Head Start/DOHMH regulations, • Assisting families in accessing and utilizing community resources, including arranging transportation when necessary, • Facilitating and supporting the participation of home-based families in program governance and program planning and assessment, • Record keeping and reporting to ensure compliance with Head Start regulations and high quality programming, • Helping families as necessary to arrange and keep medical and dental appointments required by EHS, • Working collaboratively with supervisor and program specialists/consultants (health, nutrition, mental health, disabilities and education) to ensure achievement of family goals, school readiness, and program compliance, • Active, ongoing participation in program recruitment and intake efforts, • Maintaining flexible schedule, including some evenings and/ or Saturdays, • Attending and participating in professional development, staff meetings, and reflective supervision sessions, • Ability to lift 35 pounds Please submit a resume and thoughtful cover letter, outlining how your skills and experience meet the qualifications of the position you are applying for. Job DescriptionDescription: Essential Duties/Responsibilities: • Assist with planning for program needs and department needs assessments for preschool and birth to 3 programs., • Orient and mentor therapists /clinicians, particularly early career professionals in developing clinical skills, judgement, learning and applying effective parent coaching strategies., • Coordinate CFY and graduate intern supervision, • Maintain a small caseload., • Oversee clinical scheduling and caseload assignments in collaboration with the EI and preschool Coordinators, • Ensure that therapists know and implement best practices for monitoring auditory, speech, and language developmental milestones for students, as well as counseling, education and training for parents, family and/or caregivers for in person and virtual service provision., • Assist in developing or improving procedures related to onboarding new families, communication with outside therapists or audiologists, and practices to support enrolled families in collaboration with EI and preschool coordinators., • Perform clinical review of session notes, progress reports, and evaluations as well as other internal documentation for services provided at Clarke NY., • Oversee use of Easytrac,, ensure compliance with Medicaid in Education/School Supportive Health Program, IEPs and provision of service mandates, continuity of learning plans, and compliance with regulations and contractual requirements for services, • Assist with audit preparation related to Clarke NY Programs, • Provide ongoing support for staff by conducting observations and meetings with staff to assist them in the development of their general clinical skills and use of LSL strategies. Other Duties: • Lead evaluation protocol development, training, and implementation, • LSL mentor and training coordinator, • Coordinate parent workshops on related topics, • Coordinate and participate in 1:1 meeting with therapists, EI team meetings, speech case conferences, case reviews and parent meetings Required: • New York State licensed SLP and certified TSLD, • ASHA certificate of clinical competence (ccc-SLP), • Supervision and mentoring experience, • Experience with NYSED regulations, CPSE processes, EasyTrac and School Supportive Health Programs, • Experience with NYS and NYC Early Intervention Program, services, and regulations, • Experience with parent coaching best practices, • Experience with birth-5-year-old child development, • Listening and Spoken Language (LSL) certification, • Knowledge of the CPSE, turning 3 and turning 5 processes
