Job Description Lead MST Coach If you are passionate about working with/for vulnerable youth and families, implementing evidence-based models of treatment and care, we have a professional opportunity for you! Education: MSW or mental health degree in relevant field, required w/ 2-years of professional experience, to include some knowledge of child welfare working with at risk youth;. Former case management experience is a plus. *The position also requires a valid NYS driver's license Brief Overview: The Lead MST-FIT Coach primarily provides supervision over MST Coaches that perform case management/ administrative duties around the resident youth's progress towards transition from the agency into independence, utilizing the MST-FIT proven model of service. This role is integral to the agency, department and youth of the RTC program. The Lead Coach supervises MST Coaches working within the community to coordinate/ensure appropriate resources and services are assigned and implemented for the youth's success. Job Description Client Care Coordinator Supervisor PRIMARY FUNCTION/PURPOSE: Supervise the delivery of services provided by Client Care Coordinators (Social Workers) to enhance existing services to homeless families. This includes the completion of a thorough assessment, and the development and completion of a bio-psychosocial for each family upon intake, crisis intervention, counseling, client advocacy, and referrals to government and/or community-based resources. If needed, this individual will ensure that recommendations of achievable goals be implemented on each family’s service plan, leading to permanent housing, and strengthening the overall permanency outcome for clients in shelter. MINIMUM QUALIFICATIONS: • Must be a Licensed Clinical Social Worker (LCSW) in the state of New York upon hire or within three (3) months of hire, • Minimum of 3-5 years of experience in social services, counseling, crisis intervention, family services, preventive services, housing services, or homeless services, • At least two years of supervisory experience in social services, counseling,
