Certification Details • BLSJob Details, • As an integral part of a team, the Virtual Care Coordinator supports virtual patient care activities and promotes patient-focused care within the Mount Sinai Health System., • Responsible for remote monitoring duties including patient safety and clinical needs monitoring, supporting hospital-based telemedicine services, and monitoring hospital environmental status., • Collaborates on implementing telehealth and remote patient monitoring programs, coordinates training, and participates in telehealth best practices development and execution., • Communicates and collaborates with clinical teams, patients, digital technology partners, engineering, telehealth platform vendors, and managerial staff., • Shares accountability for clinical quality and safety outcomes., • Performs quality assurance and peer reviews, evaluates telehealth and virtual patient observation programs, and recommends improvements., • Meets regularly with senior leadership to discuss Virtual Hospital Program quality metrics and workflow expansion., • Leads case reviews with hospital partners and implements solutions to monitor outcomes., • Triages and assigns cases for virtual monitoring/observation., • Collaborates on telehealth policy and workflow development and implementation., • Maintains operational database for telehealth and remote monitoring program performance and quality metrics., • Identifies trends and participates in corrective action plans., • Trains staff on telehealth workflows, platform application, and documentation requirements., • Serves as liaison between physicians, clinical staff, digital technology partners, and vendors regarding telehealth platform issues., • Facilitates camera utilization and coordinates patient criteria reports with hospital care teams., • Navigates multiple technology platforms to ensure quality care and best outcomes., • Provides real-time support to physicians and clinical staff for telehealth and remote monitoring platforms and equipment., • Observes patients continuously for changes in clinical status or behavior to ensure safety and prevent falls and injury as directed by registered nurses., • Performs clinical early warning sign monitoring such as vital signs, central line days, and Foley catheter days., • Communicates effectively with patients, visitors, and clinical team members., • Reports any variations or concerns in patient status to registered nurse or physician immediately., • Documents patient behavior, redirection interventions, and escalations to clinical team., • Suggests discharge from remote patient monitoring based on observed patient behavior., • Monitors Epic telehealth consult orders, scheduled consults, and follow-up telehealth visits., • Ensures telehealth sessions start and complete as scheduled without issues., • Greets patients, family, and visitors and introduces self and role., • Answers calls promptly from clinical teams and uses approved communication methods., • Assists in interpreting through approved methods as needed., • Protects patient privacy by adhering to information security policies and HIPAA guidelines., • Escalates and reports patient safety concerns and telehealth equipment issues to appropriate leadership., • Maintains accurate records of interventions and safety events., • Sets priorities, problem solves, uses proper judgment, and remains flexible in difficult situations., • Communicates effectively in English both orally and in writing., • Submits utilization and assignment reports every shift to supervisor and nursing leadership., • Attends staff meetings, checks work email regularly, and participates in team briefs., • Maintains attendance and punctuality records according to department policy., • Maintains a safe, clean, and functional environment and reports physical plant and equipment problems to managers.Job Requirements, • Minimum three years of direct patient care experience., • Experience creating policies impacting telehealth., • Experience with quality, outcomes, and data collection., • Ability to analyze legislative, regulatory, and organizational policies affecting telehealth., • Ability to identify data elements for quality improvement and assurance., • Experience developing efficient clinical telehealth workflows., • Experience drafting organizational protocols to support telehealth., • Experience developing strategies for effective telehealth program development., • Ability to measure and communicate program successes and lessons learned., • Experience identifying changes in patient clinical status and behavior requiring escalation., • Experience identifying patient behaviors that place patients at safety risk., • Successful collaboration with interdisciplinary teams., • Experience with telehealth or remote monitoring technology., • Experience with electronic medical records (EMR)., • Experience with telehealth technologies and their appropriate uses., • Experience identifying benefits and barriers to telehealth implementation., • Experience communicating telehealth concepts to organizational members and healthcare consumers., • Ability to identify elements of successful telehealth encounters and quality telepresence., • Ability to consider language, culture, and disability in telehealth technology use., • Associate or Bachelor’s degree in a health-related field preferred.Schedule Information, • Shift includes 7 hours per day., • Total hours per week: 37.5., • Requires 2 years
