Job Description The Franklin House Transitional Housing Program provides housing for homeless women and their children in a home like atmosphere. The families are each provided a private and comfortable living space in which they reside for approximately 12 months. During this period of time, the women are involved in educational programs, build work-readiness skills, secure employment, gain parenting, financial and other daily living skills. Staff are on-site 24/7 to provide services and support as needed. Desired Qualities of a Case Manager: • Positive communication, • Excellent supportive counseling skills, • Compassion, • Reliability & dedication, • Desire to make a difference, • Sound ethical and moral values, • Professionalism, • High school degree required, Associates or Bachelors degree preferred, • Ability to pass criminal/registry background checks/drug screen, • A valid driver's license, no more than 5 points or DUI in last 3 years, • Ability to pass and maintain required Agency and program- specific trainings, • Experience working with the homeless population preferred Essential Duties and Responsibilities of a Case Manager: • Assist clients in developing goals oriented towards gaining self-sufficiency, parenting skills, finding permanent housing, etc., • Provide on-site services and support, including provision of direct service to clients in areas such as money management, daily living skills, psycho-education, work readiness skills, etc., in order to be able to transition into permanent housing, • Provide transportation as needed to assist with child care, grocery shopping and other needs in the community., • Support families personal outcomes to enhance quality of life, • Utilize an electronic health record (EHR) system to write clear and concise progress notes, • Ability to pass all Agency and program specific trainings Training: • All employees are required to complete 10-15 trainings upon hire and on an ongoing basis., • Trainings are a combination of in person, which require staff to commute to the main office in Raritan NJ, and remote, using Go To Meeting., • All trainings are paid.
