Job Description Company Overview: Public Health Solutions (PHS) is a 501(c)3 non-profit community-based organization (CBO) that has existed for 70 years to improve health equity and address health-related social needs (HRSN) for historically underserved marginalized communities. PHS administers WholeYouNYC (WYNYC), a coordinated community resource network that builds trustworthy and reliable pathways between healthcare providers, health plans and CBOs providing critical resources in the community that address the social drivers of health. WYNYC brings together over 100 organizations offering various programs – such as food, housing, employment, health insurance, and sexual health services – across all five boroughs. These services and programs make it possible for New Yorkers to live their healthiest lives and ultimately reduce health disparities and advance health equity. To date, our network has already impacted thousands of lives through community partnerships and referrals, generating millions in estimated healthcare savings. New York State (NYS) recently announced the availability of $500M statewide to support Social Care Network (SCN) lead entities responsible for coordinating social care delivery in various regions across the state. Public Health Solutions (PHS) and our WYNYC network were awarded the role of regional SCN for Brooklyn, Manhattan, and Queens. This is a grant-funded position ending March 31, 2027. Specifically, the Project Manager, Technology Strategy will: • Manage programmatic relationships with referral platform and other technology vendors, troubleshooting issues and managing enhancements ensuring underlying network technology solutions facilitate network efficiency and effectiveness., • Coordinate internal review of deliverables for timely submission and organized tracking of project activities., • Work with internal teams and network partners to determine needs within the network, including identification, tracking, and reporting of bugs or challenges in technology systems., • Support and lead change management initiatives to ensure smooth transitions and effective adoption of new WholeYouNYC processes, policies and procedures., • Lead discussions with internal teams and external partners on service eligibility, community resource network use and best practices, troubleshooting network issues as they arise., • Work collaboratively with Quality & Evaluation to manage, analyze, and report on WholeYouNYC data and performance., • Develop and implement network policies and procedures, workflows and guidance documents that enhance network effectiveness., • Evaluate and optimize WholeYouNYC workflows ensuring seamless integration across the network., • Drive communication and information management of technology platform updates among internal staff and with network partners., • Collaborate to refine and optimize the data platform to streamline and enhance the HRSN service invoicing process., • Perform audits, quality assurance (QA), and reconciliation of platform-generated invoices to identify potential data gaps and opportunities for platform improvements., • Support relationships with WholeYouNYC network partners ensuring continued participation in the network, as needed., • Plan and deliver trainings on WholeYouNYC social service offerings and best practices, as needed., • Lead and conduct audits of network activities ensuring adherence to business standards, partner and government requirements, as needed., • Bachelor's degree from four-year college or equivalent., • Prior work experience in project management, reflecting a strong understanding of project management methodologies., • A strong familiarity with and understanding of the public health, social services and healthcare landscape in New York City., • Self-motivated, highly organized with the ability to work autonomously., • Excellent presentation, communication and interpersonal skills., • Proven experience leading cross-functional teams to successful outcomes., • Detail oriented possessing strong analytical and problem-solving abilities., • Proficiency in Microsoft Office Suite tools (e.g., Word, Excel, PowerPoint)., • Experience with data collection, reporting, and analysis., • Excellent team player with the ability to be flexible, work collaboratively and respectfully both internally and externally and engage in consensus-based decision making., • Dedicated to helping improve the lives of disenfranchised and marginalized communities., • Ability to thrive in a work environment that is respectful, goal-oriented, and focused, and yet is dedicated to achieving work-life balance., • Experience working with or within a technology platform preferred., • Master’s degree preferred., • Bilingual in Spanish or multilingual., • Prior experience in project launch, partnership building and network coordination. • Hybrid Work Schedule., • Generous Paid Time Off and Holidays., • An attractive and comprehensive benefits package including Medical, Dental and Vision., • Flexible Spending Accounts and Commuter Benefits., • Company Paid Life Insurance and Disability Coverage., • 403(b) + employer matching and discretionary company contributions., • College Savings Plan., • Ongoing trainings and continuous opportunities for professional growth and development.
