This role involves assessing client needs, developing personalized service plans, and coordinating resources to help clients achieve stability and self-sufficiency. Key Responsibilities: Conduct thorough assessments of clients' needs, strengths, and barriers. Develop and implement individualized service plans in collaboration with clients. Minimum of 2 years of experience in case management, preferably in a homeless shelter or similar setting. Strong understanding of issues related to homelessness, poverty, and mental health.
