Job Description Form Position Title: Senior Rehabilitation Technician Location: various Reports To: Habilitation Specialist, Rehab Specialist Cost Center: various General Purpose: Assist Habilitation Specialist or Rehabilitation Specialist in the supervision of Rehabilitation Technicians at their assigned sites, in meeting program goals, and overseeing work quality Essential Functions: * In absence of supervisor, ensure Rehabilitation Technicians are following their schedules and are at their assigned sites * Ensure that staff are documenting services as per funding source regulations (i.e. Medicaid and DQA) * In conjunction with supervisor to deliver and record the implementation of services as indicated on the Individuals’ Habilitation Plans * Assist supervisor in ensuring Rehabilitation Technicians are providing services as designed in the Hab Plans * Monitor individual’s behavior and intervene as appropriate and inform supervisor * Work as part of the team to ensure individuals receive seamless services provision in a variety of appropriate activities throughout the day. * Work with supervisor or Activity Instructor to lead and coordinate daily community integration activities * In conjunction with supervisor create schedules that are based on individuals needs, services, and staff availability * Maintain contact with volunteer sites (site cancelations…etc.) and update Volunteer Site list regularly. * Liaison with job coaches, individuals families, and caretakers * Collect and record performance data from internship, volunteer, community sites * Monitor and supervise individual sign in and out of program and ensure that information is accurately recorded in AWARDS * Ensure compliance with DOL regulations especially around volunteering in for profits * Report all incidents as mandated by OPWDD and NYS Regulations * Conduct Monthly fire drills at each Hub site in the absence of supervisor * Order supplies for each site in the absence of supervisor. * Obtain and distribute metro cards for staff and individuals at each site. In absence of supervisor pick up and handout staff metro cards. * Responsible for petty cash Qualifications/Basic Job Requirements: Education: HS Diploma/GED required, Bachelor degree from an accredited school in related field preferred. Experience: Six months in mental health, vocational rehabilitation, or case management preferred. Skills: Strong interpersonal & communication skills. Strong organizational and time-management skills. Ability to handle multiple tasks simultaneously and prioritize demands. Ability to work independently with minimal supervision. Flexible and open to new ideas. Approaches problems in a solution oriented manner. Avoids making assumptions about individual’s abilities and motivation. Deals with confidential information and issues using discretion and judgment. Scope of Responsibility & Positions Supervised: * Oversee the performance of the Rehabilitation Technicians on a daily basis. * Make decisions on schedule changes for individuals and staff in conjunction with supervisor. * Address performance issues with Rehabilitation Technicians and provide training. Position Salary: $26 / hour, Part-Time (35 Bi-weekly) Position Summary The Peer Specialist is responsible for working with Counseling & Crisis Center recipients in developing a plan to advance recovery goals and manage crises as they occur. The Peer Specialist will facilitate linkages to available community-based supports that address physical health, behavioral health and social determinants of health. The Peer Specialist will work as part of the Rehab & Recovery programs using the Intentional Peer Support model of care to help recipients define and manage their recovery. Job Description JOB SUMMARY: The Clinical Specialist is a mental health professional who reports directly to the Division VP. This position is primarily clinical and consultative in nature and provides services to all supported housing programs (both single- and scatter site) throughout the cluster. While the Clinical Specialist does not carry formal supervisory or operational responsibilities, the VP may deputize him/her to delegate and monitor certain tasks and responsibilities to staff at the Program Director level and below in relevant Supported Housing (SH) programs. ESSENTIAL JOB FUNCTIONS: • Assists Program Directors and case management staff to provide quality clinical and crisis management services across the supported housing system, primarily through consultation and training., • Provides consultation to staff on challenging clinical cases, such as those involving high-risk consumers, (i.e., those with co-morbid medical conditions, those identified as at risk psychiatrically) or individuals who present compliance issues or added complexity due to factors such as AOT/forensic status. The Clinical Case Management Specialist will ensure a smooth transition for Adult Home Residents moving into Supported Housing via the Impacted Adult Home Initiative//Settlement. Attend care coordination meetings, collaborate with ICL in-reach staff and external care coordinators, assess consumer needs, and discuss/advocate for needed services. Maintain adherence with settlement agreements regarding consumers moving into ICL units. Travels to/visits consumers' residences or apartments to provide counsel and assistance to help the consumers, within their capabilities and interests, maintain the most significant degree of independent living. Knowledge of the principles and practices of harm reduction. Substance abuse counseling experience with individuals with co-occurring mental illness and substance abuse. Experience in providing case management services to individuals with mental illness as defined above. Experience providing clinical consultation and supervision in a community mental health setting. Knowledge of integrated mental health and substance abuse assessment and treatment, motivational interviewing techniques, and stage-wise treatment.
