Our Techs are responsible for all the pre-exam elements of the patient visit. This includes gathering and documenting patient information and history and conducting routine and ancillary patient testing. You’ll play a vital role in the patients experience with OCLI and in helping our Dr’s to diagnose and treat eye disorders. OUR EXPECTATIONS OF YOU • Tests and measures eye function to assist Ophthalmologist to diagnose and treat eye disorders and disease, • Performs and records initial history; including present/chief complaint, past ocular history, family history, systemic illness, medications, allergies and drug reactions, • Tests and records patient's far acuity, near acuity, peripheral vision, depth perception, and color perception, • Measures intraocular pressure of eyes (glaucoma test), excluding applanation, • Performs ancillary test such as lensometry, corneal topography, A scans, B scans, keratometry, fundus photography, slit lamp photography, specular misoscopy, and visual fields, (including central and peripheral vision); records results, • Transcribes physician's dictation into patient chart when applicable, • Knowledgeable in ocular pharmacology such as anesthetics, steroids, antibiotics, mydriatics and cycloplegics, epinephrine, miotics and non-steroidal anti-inflammatories, • Applies drops to anesthetize, dilate, or medicate eyes, • Collects and processes specimens for appropriate laboratory testing, • Prepares and stocks exam rooms; assists with exam, • Triages emergency cases appropriately into physician’s schedule, maintains patient flow, • Prepares patient and procedure room; assists ophthalmologist with in office procedures, • Performs first level maintenance on ophthalmic equipment and notifies office manager of any equipment malfunctions, • Utilizes as appropriate, infection control techniques and universal precautions, • Performs other tests and measurements as requested by Ophthalmologist, • Maintains confidentiality and professional demeanor at all times WHAT YOU'LL NEED TO SUCCEED • You have one-year certificate from college or technical school; minimum two years experience as an ophthalmic assistant or equivalent combination of education and experience., • You are CPR Certified, • You’re an excellent communicator – both verbally and written, • You’re outgoing, you love the idea of interacting with patients everyday, • You’re comfortable providing white glove service and responding quickly to our patients needs, • You’re a dependable problem solver – you’re the one people call to keep cool in stressful situations, • You have a positive, can-do attitude and work well in collaborative environments, • You’re flexible – you roll with the punches and easily accept change, • You’re knowledgeable in ocular pharmacology such as anesthetics, steroids, antibiotics, mydriatics and cycloplegics, epinephrine, miotics and non-steroidal anti-inflammatories is a PLUS, but not a requirement, • While performing the duties of this Job, the employee is regularly required to stand; walk and use hands to finger, handle, or feel. Job DescriptionDescription: Care Management is a service model designed to ensure comprehensive communication and coordination among all caregivers involved in a resident's care. A designated "care manager" takes the lead, overseeing and providing access to the necessary services to address the resident's needs, preventing hospitalizations, maintaining health, and ensuring stability. The Care Manager is responsible for providing and coordinating services for their assigned caseload. This involves guiding program enrollees and their legal guardians through the healthcare system, addressing access issues, building relationships with service providers, and monitoring interventions and outcomes. Requirements: Bilingual in Spanish & English A Must Ø Obtain required Care Management enrollment consent from individuals or legal guardians. Ø Conduct initial and ongoing needs assessments (Child and Adolescent Needs and Strengths; CANS) to determine the most appropriate level of care management. Ø Manage the patient’s Individualized Plan of Care, coordinating services, supporting treatment adherence, and monitoring various needs, including prevention, wellness, medical mental health, care transitions, and social and community services. Ø Meet client contact requirements, including face-to-face visits based on acuity levels. Ø Fulfill Care Management documentation requirements using designated tools like Foothold Care Management HER (FCM) and Health Commerce Systems (HCS) such as UAS-NY and MAPP applications Ø Serve as an advocate for clients within the agency and with external service providers. Ø Promote wellness and prevention by connecting enrollees with resources based on individual needs and preferences including but not limited to ameliorating gaps in care and managing medical/dental appointments. Ø Educate children/caregivers on the care of chronic conditions, immunization, screening, and preventive interventions. Ø Assist clients in obtaining and maintaining public benefits essential for healthcare services. Ø Effectively communicate and share information with individuals, families, and other caregivers, considering language, literacy, and cultural preferences. Ø Identify available community-based resources, complete referrals, promote engagement and follow-up of implemented services. MINIMUM QUALIFICATIONS: Ø Bachelor of Arts or Science with two years’ experience (Complex/Intense Acuity members) PREFERRED EXPERIENCE: Ø Valid driver’s license with a good driving record. (Plan to attain within 60 days of hire) Ø Efficient knowledge of/familiarity with CANS 2.0 assessment and Care Management software (FCM/UAS-NY/MAPP) PREFERRED SKILLS/REQUIREMENTS: Ø Proficiency in computer software packages (MS Word, Excel, Outlook, Google office, MS-Teams) Ø Excellent oral and written communication skills. Ø Care Managers serving high acuity enrollees should demonstrate knowledge and understanding through additional experience, education, or training. Ø Proven self-management abilities, meeting deadlines, ensuring compliance with agency policies, and overseeing complete and timely maintenance of agency records. Ø Care Managers for children with medical fragility must have extensive experience coordinating their care. Ø Experience providing service coordination, information, linkages, and referrals for community-based services.
