Via Vyse is a building which was developed and is operated by Dunn Development Corp., a private developer. is a newly constructed residential building located in the East Morrisiana section of New York City. This new construction is approximately 102,000 square foot within a 12-story elevator building located at 1812 Vyse Avenue in The Bronx. The project has 121 residential dwelling units, one of which will be a superintendent's unit. The unit distribution is 46 studios, with the remainder being one and two bedroom units. CAMBA, Inc. is the on-site services provider at Via Vyse. Sixty one apartments, 51% of the units in the building, will be master leased by CAMBA and will house chronically homeless single adults and families who, the head of households, are diagnosed with chronic health conditions. The remaining 59 units are occupied by community residents, who may require some services. Position: Bilingual (Spanish) Case Manager Reports To: Program Manger Location: 1812 Vyse Avenue Bronx, NY 10460 What The Bilingual (Spanish) Case Manager Does: • Review all documentation establishing tenants' eligibility for program and make file copies., • Create and maintain tenant files., • Conduct initial intake and assessment of tenants and tenants' families' needs and periodic re-assessments., • In collaboration with tenants, prepare initial and periodic revisions of independent living plans including short-term and long-term tenant goals., • Assist tenants in attaining their goals by identifying and locating community resources for tenants and by making referrals to appropriate services both within and outside CAMBA., • Work with tenants to break through barriers to tenant goals and to assist tenants in advocating for themselves and in moving toward self-sufficiency., • Conduct group sessions and/or workshops to assist tenants with independent living skills., • Recommend and implement strategies to persuade tenants to participate more fully in this process., • Monitor tenants' progress toward their goals via regularly scheduled telephone contact and/or face-to-face visits, and document via progress notes., • Participate in case conferences., • Follow-up with tenants and with referral organizations regarding tenant contact and progress with referral organization., • Provide all required information for weekly/monthly/quarterly/annual reports., • May recommend closing of cases in which tenants have: (a) achieved primary goals and have maintained stability for a period of months; or, (b) have not demonstrated a willingness to participate in the process (lost-to-service); or, (c) have become ineligible for services (e.g. moved out of building), • May schedule appointments for tenant with referral organizations., • May assist tenants in completing applications for benefits and entitlements, and may process applications on tenants' behalf., • May follow-up with tenants for a period of time after successful completion of their primary goals to assure tenant stability., • May evaluate actual living conditions of tenants through home visits. Job Description The role… At Soho House, a Busser is responsible for curating a we organized, clean and structured member and guest experience. A Busser, supports runners, servers and management by cleaning and resetting tables, ensuring the availability of flatware and plates, and bringing guest needs to the attention of waitstaff. They also restock necessary items, such as napkins and straws, and may assist with washing dishes. A successful Busser will be efficient, attentive, and have strong customer service skills. Job Description Senior Case Manager - Brooklyn Criminal Court Summary: The Senior Case Manager provides direct assistance, either in-person or telephonically, to victims of crime and abuse, specializing in working with survivors of intimate partner and family violence (includes sexual assault and families of homicide victims). The Senior Case Manager will provide services to victims of crime and abuse. Responsibilities: • Learn and integrate a trauma-informed, client-centered approach, using an anti-oppressive lens, into your work with clients., • Conducts client centered safety assessment and risk management planning;, • Provides crisis intervention and direct assistance to victims of crime and abuse; develop creative solutions reflecting clients’ unique needs in collaboration with client, • Advocates for clients for public entitlements with systems and other agencies, where necessary, such as obtaining orders of protection, help with hospital/medical related needs; Office of Victims Services compensation, and relocation assistance;, • Provides information and referrals and facilitates linkages for clients for services such as shelters, mental health treatment, public assistance and other entitlements based on assessment and clients’ needs;, • Assesses for and provides emergency practical assistance for clients including transportation, food vouchers, financial assistance and lock replacement in accordance with agency policy;, • Engages in regular supervision meetings with supervisors to review cases, reflect on client interaction and build skills;, • Maintains thorough and accurate case records and statistics in web-based case management system;, • Collects client data, tests new interventions and tracks outcome;, • Participates in staff meetings, individual and group supervision, community meetings and relevant trainings and seminars;, • Conducts educational presentations and trainings and participates in outreach events for community groups about intimate partner and family violence, victim assistance resources and other related topics;, • Works collaboratively with other Safe Horizon programs to facilitate linkages for clients;, • Safe Horizon staff, • Criminal justice personnel, • District Attorney’s Office, • NYPD, • Office of Victim Services, • ACS, • NYCHA and Section 8 staff Knowledge and Skills: • Must be sensitive towards and understanding of victimization;, • Must be able to demonstrate respect, compassion and a non-judgmental approach to victims of crime and abuse;, • Must be willing to participate in a learning community;, • Must be well organized and detail oriented;, • Must demonstrate the ability to work independently and be resourceful;, • Must demonstrate leadership skills (acts as a “go-to” person for colleagues, takes initiative; suggests ways to make processes work better);, • Must be a team player;, • Possess strong communication and organizational skills;, • Excellent interpersonal skills – must have the ability to be assertive; and, • At least three years of prior direct service experience in a social service setting is required – Or- a minimum of two years of case management experience at Safe Horizon is preferred.
