Job Description Summary: Streetwork Project Lower East Side is an innovative program for homeless youth providing drop-in, crisis shelter, and outreach, utilizing a harm reduction approach. Under this umbrella, Senior Case Managers provide comprehensive services to adolescents and young adults impacted by homelessness and involved in street activities (gangs, violence, drug use/sale, and survival sex). Services provided by Senior Case Manager include assessment, case planning, case facilitation and management, counseling, referrals, outreach, crisis transportation, daily living services (food, shower, respite), triage counseling, recreational activities and support groups. The Senior Case Manager also works alongside program leadership in providing input towards program planning to meet the mental health and substance use harm reduction needs around staff development and client services. Senior Case Manager coordinates with Safe Horizon supporting programs and local agencies to provide short- and long-term care necessary to meet the wide range of needs related to homeless adolescents. Responsibilities: • Learn and integrate a trauma-informed, client-centered approach, using an anti-oppressive lens, into your work with clients., • Intake and assessment for individuals seeking assistance, • Triage counseling, • Mediation for internal conflicts, • Engagement of adolescents into program structure (harm reduction philosophy, rules, mutual respect), • Support Drop-in staff in providing youth access to daily living needs (food, clothes, hygiene supplies, space to rest), • Provide supportive and crisis intervention counseling utilizing a trauma-based intervention (ARC) for youth with more complex mental health needs and challenges., • Provide trainings on overdose prevention, Narcan, as well as provide syringe exchange and other harm reduction supplies with clients, • Collaborate and coordinate with on-site mental health providers to ensure clients access to care., • Work in collaboration with therapist and Supervising Social Worker (SSW) to help develop treatment plans for youth with special needs experiencing severe mental health challenges, • Provide assistance to SSW in facilitation of clinical group process and debriefs, • Collaborate with SSW in identifying trainings around staff development, • Provide support to Supervising Coordinators and Coordinators in absence of directors around scheduling and oversight of the space., • Serve as an advocate and liaison for clients negotiating the shelter system and other social service programs, • Enrollment of clients for Public Assistance & Food-stamps, • Coordinate with social worker in assessing clients’ eligibility and assist with processing for SSI benefits, • Assist youth in application for Supportive Housing and SSI., • Complete psychosocial reports on youth for Supportive Housing & SOAR screening tools for SSI, • Advocacy on behalf of under 18-year-old clients for Administration for Children’s Services support, • Facilitate self-care and coping and risk reduction support groups, • Provide group led facilitation and conducting of educational presentations on HIV/AIDS, risk reduction, and other topics related to sub-populations of homeless youth, • Conduct outreach to homeless and street involved young people, as assigned, • Assist in community outreach and trainings on harm reduction techniques, including Narcan training; overdose prevention; fentanyl testing, • Escort homeless youth to appropriate services and provide transportation as part of case plan, • Preparation of case records, prepare monthly and quarterly statistical reports, • Maintain routine administrative responsibilities to include the preparation of correspondence, maintaining files, mail, email, and telephone., • Participate at staff trainings, staff meetings and staff group process workshops & case conferencing, • Participate in weekly supervision. Secondary Job Functions: • Petty Cash reconciliation, • Identification of new resources appropriate for population, • Provide wellness and recreational (engagement) activities, • Coordinate services with other Streetwork staff and on-site legal, medical, and psychiatric service providers Interacts with: • All Streetwork program sites and staff, • All internal Safe Horizon programs and departments that can comprehensively support the client and case plan, • All external agencies and organizations that can provide appropriate services to the client Knowledge and Skills Required: • Compassionate approach to others and openness to new ideas, • Dynamic energy, able to multi-task and negotiate multiple demands on attention, • Non-judgmental approach and ability to empathize with others, • Clear and concise verbal and written communication skill, excellent interpersonal and organizational skills, • Genuine interest in working with disenfranchised youth and young adults, • Understanding of social service systems and how to provide advocacy for clients navigating systems, • Basic knowledge of Supportive Housing and SSI eligibility process, • Ability to complete psychosocial reports and conduct assessments using psychiatric reports Qualifications: • Bachelor’s Degree, creative arts, liberal studies or any field related to working with youth +2 years’ experience working with youth, or High school diploma or equivalent + minimum of 5 years’ experience working in social service settings with target population. OR, two years of high-quality work at a Streetwork Project site Desirable: • Personal experience with street survival activities (homelessness, gangs, drug economy, sex industry) with three years separation from involvement in these activities. Deep understanding of the relationship between personal experience and professional standards., • Understanding of harm reduction perspective, • Experience working with youth who have Co-occurring Disorders, • At least 1 year working with the homeless youth, or harm reduction, • Bi-lingual Spanish Other: Basic understanding of computer and systems for managing case records. POSITION OVERVIEW Under supervision of the Program Director, the Case Manager is responsible for providing case management, housing support to participants and monthly reporting requirements that will ensure compliance with the parameters of the program. KEY ESSENTIAL FUNCTIONS • Provide targeted case management services to children, youth and adult consumers living with mental health illnesses and substance abuse disorders and their families/support systems through ensuring access to care, engagement in care coordination of care to obtain the full range of needed services., • Gather enrollment consents, PSYCKES/RHIO consents, and complete screening, baseline-risk assessments, reassessments, plan of care, plan of care updates and notes in accordance with departmental policies., • Demonstrate the ability to clearly articulate, verbally and in writing, the aims and goals of the department to potential patients, community members and staff., • Participate in quality improvement activities, projects and reviews in collaboration clinical team members., • Complete daily, weekly, monthly, or other periodic requests for narrative or quantitative data reports for program review., • Prioritize the homeless population through identification of new sources of potential patients, onsite meetings with patients at their shelter and conduct outreach and engagement presentations., • Meet regularly with supervisor and attend staff meetings and case conferences. Be prepared to discuss case management and operational issues impacting performance and program operations., • Complete and submit daily activity log in accordance with departmental policies., • Ensure patient is attending scheduled medical and social service visits through building relationships with patients and providers. Coordinate and schedule appointments with Social Worker and Medical/Mental Health providers. Routine calls should be made to internal and external providers before and after visits to follow up and provide necessary support to the patients., • Maintain four contacts with each client or at a greater frequency as indicated by the risk stratification and plan of care., • Access and respond per agency guidelines to client complaints of grievances, • Conduct outreach and engagement in accordance with policies via phone, electronic methods, and letter and or field work to client/collateral/provider to engage clients or strengthen connectivity., • Help maintain health and wellness and prevent secondary disease complications through provision of health information, support plan of care, and coaching., • Promote and expand linkage development in all areas related to patient care including social determinants (e.g. housing, employment) and monitor the effectiveness of linkages with other service providers via phone, face to face meetings of formal case conferences., • Communicate effectively with Supervisor in identifying strengths, weakness and opportunities of program operations., • Ensure community-follow up to engage the client in care; promote compliance with medical appointments and encourages client self-sufficiency and empowerment., • High School Diploma or GED required., • Associate Degree and Bachelor’s degree preferred., • Minimum of six (6) years of experience in case management., • Must obtain Mandated reporter (2 hours) training/certificate required., • Must complete Integrated Mental Health/Addictions Treatment Training (IMHATT) Certificate within three (3) months of hire date., • Must complete Integrated Mental Health/Addictions Treatment Training (IMHATT) Certificate, • Must be trained in Trauma Informed Care and in Military and Veteran’s culture, • Must be team oriented and possess a positive attitude with a willingness to be flexible and helpful., • Must be able to multi-task with strong organizational skills., • Excellent interpersonal skills and able to communicate both verbally and in written form., • Commitment to cultural diversity and sensitivity.
