Job Description BESTCARE is an industry leader in providing quality home care to the members of the communities we serve in New York City, Nassau, Suffolk, and Westchester counties since 1981. We are seeking a Full time RN FIELD SUPERVISOR who will be based at our Yonkers location. As a field supervisor, you will further Bestcare’s goal for our patients to function safely, at their optimum level of independence. Through education, communication, empowerment, and technology we are focused on transformative care while optimizing patient and employee satisfaction. Job Responsibilities: · Conduct initial home visits to assess patient needs and develop the Aide Plan of Care · Communicate with a patient’s physician and other team members to ensure delivery of appropriate patient care · Supervise patient care provided by our Home Health and Personal Care Aides · Participate in orientation, teaching, and evaluations of care providers · Responsible for the review and maintenance of patient records, using telehealth modalities for early intervention of patient issues Job Qualifications: · Must be a valid Registered Nurse licensed in New York State, preferably with two (2) years of experience · Must have a valid driver’s license and transportation. · Previous home care experience is a plus · Bilingual Spanish and English language abilities is a plus Benefits: • FLEXIBLE SCHEDULE, • Strong team approach and support, • Laptops and tele-health technology support We are an Employee Stock Ownership (ESOP) Company, offering a competitive compensation and benefit package, including medical, dental, life insurance, and matching 401K plans Our commitment to providing consistent, quality care to our patients is the motivation for our care team.
