Job Description Case Manager - Qualification and Competencies • High School Diploma plus five years’ experience in the Human Services Field OR An Associate’s Degree in Human Services plus three years of experience in the Human services field OR Bachelor’s degree in Human service related field, • Critical thinking and sound judgment skills are necessary., • Ability to manage change in a fast-paced growing business environment., • Computer/office equipment usage, • Ability to work independently and as part of a team., • Demonstrates organizational and communication skills which would enable effective interaction with agency staff, consumers/clients/families, and the community. Schedule • Mondays-Fridays 9am-5pm (training period only), • Tuesdays-Saturdays or Sundays-Thursdays, • Minimum 1 late shift (ending at 6/6:30pm) Location • Office: Farmingdale, NY 11735, • Travel: throughout Suffolk County Case Manager - General Description Responsible for providing supportive counseling and training in independent living skills to residents of the Apartment Treatment Program. Case Manager - Duties and Responsibilities • Provides training in independent living skills, tenant responsibility, and psychiatric stability., • Coordinates admissions, transfers, and discharges., • Makes routine visits to residents., • Participates in developing, evaluating, and modifying service plans., • Supports medication self-administration; learns medication effects and side effects., • Assists with vocational/educational goals and socialization skills., • Organizes recreational activities and provides counseling/crisis management., • Ensures fire safety compliance and conducts apartment inspections., • Provides transportation as needed and distributes resident allowances., • Leads apartment meetings and coordinates treatment team meetings., • Attends staff meetings, training, and agency committees as required., • Completes all documentation accurately and on time. Physical Demands & Work Conditions • Heavy travel to sites; may transport clients in agency/personal car., • Assist with client moves and cleaning demonstrations., • Lift up to 20 lbs; possible PPE use in certain apartments.
