Job DescriptionBenefits: • Competitive salary Responsibilities: • Conduct comprehensive assessments of patients' physical capabilities, medical histories, and functional limitations to establish baseline data and formulate individualized treatment plans., • Develop and implement evidence-based physical therapy interventions aimed at improving patients' mobility, strength, balance, coordination, and range of motion., • Educate patients and their caregivers on proper techniques for performing therapeutic exercises, using assistive devices, and implementing strategies to prevent injuries and promote optimal physical function., • Monitor patients' progress, reassess goals, and modify treatment plans as necessary to ensure optimal outcomes and continuity of care., • Collaborate with interdisciplinary healthcare team members, including physicians, nurses, occupational therapists, and speech-language pathologists, to coordinate patient care and promote holistic rehabilitation., • Maintain accurate and up-to-date documentation of patient evaluations, treatment sessions, progress notes, and discharge summaries in compliance with regulatory standards and agency policies., • Provide emotional support and encouragement to patients and their families, fostering a trusting and therapeutic relationship conducive to effective rehabilitation., • Stay abreast of advancements in physical therapy techniques, technologies, and research findings, and integrate new knowledge into clinical practice to enhance the quality of care provided., • Adhere to ethical principles and professional standards of conduct, ensuring confidentiality, respect for patient autonomy, and adherence to legal and ethical guidelines., • Graduate of an accredited physical therapy program with a Doctor of Physical Therapy (DPT) or Master of Physical Therapy (MPT) degree., • Valid state licensure as a Physical Therapist, with current CPR certification., • Prior experience in home health, geriatrics, or rehabilitative care preferred., • Strong clinical assessment and critical thinking skills, with the ability to develop and implement individualized treatment plans based on thorough evaluations., • Excellent interpersonal and communication skills, with the ability to establish rapport with patients, caregivers, and interdisciplinary team members., • Compassionate, patient-centered approach to care, with a commitment to empowering patients to achieve their rehabilitation goals., • Ability to work independently and manage time effectively, prioritizing tasks and adapting to changing patient needs in a dynamic home health environment., • Proficiency in documentation software and electronic health records (EHR) systems., • Willingness to travel to patients' homes within designated service areas and maintain a valid driver's license and reliable transportation., • Commitment to upholding ethical standards and complying with regulatory requirements governing physical therapy practice.
