We do this by providing Mental Health Advocacy and Peer Support, Homeless Outreach Programs and Services, and HCBS services, and Care Management to those in need Purpose of the Role: Serving persons with mental health conditions, substance abuse issues and/or disabilities, responsible for day-to-day client interactions and care coordination of assigned cases. Address immediate and emerging needs, set goals, resolve issues, advocate and connect clients to the needed resources such that their lives become stable, recovery centric and interactive. Enhance coordination of medical and behavioral health care with the main focus on the needs of persons with multiple chronic illness. Essential Functions of the Role: • Assess documentation for compliance as well as assessing staff performance and accountability, • Assess notes for quality and attentiveness, determine whether there is full engagement and determine whether assistance is needed with direction and resources, • Assist as needed with referrals to community linkages and social support services such as obtaining benefits and entitlements, food, shelter, permanent housing, • Assist individuals to help connect to natural supports that enhance the quality and security of life, • Assist Staff with developing crisis diversion plans or relapse prevention plans, • Assist/coach peer support workers in how to best engage in both self-care and client care in an effective way with a goal of maintaining recovery and stability, • Attend all trainings and meetings as assigned, • Conduct monthly meetings per OMH and County guidelines, • Create a venue for peer support staff to talk about their experiences in working with our population and whether/how it impacts their MH and recovery process, • Increase staff awareness of existing services, pathways to recovery and helping a person to remove barriers that exist for access to them, • Meet with team members to provide case direction and strategies, • One to one peer support using person centered goal planning that incorporates life areas such as community connectedness, physical wellness, spirituality, employment, self-help, • Outreach and Engagement: Companionship and modeling of recovery lifestyle, including participation in recovery activities that might be beyond the scope of treatment providers (e.g., eating together at a restaurant, attending or participating in a sporting event, attending a social event such as a concert or recovery celebration event), • Providing peer support in the person’s home or in the community to support them before (or in) a crisis or relapse, • Recovery Supports: Recovery education and coaching for individuals and their family members., • Remain passionate about quality client care, relationships and interactions with staff, • Use lived experience to draw empathy and to model recovery while ensuring the interactions are focused on client needs and client focused., • Bilingual English/Spanish a plus., • Computer literacy required., • Minimum 3 years’ experience, • NYCPS (New York Certified Peer Specialist). Permanent Certification preferred., • Tenacity and passion for this work with the ability to balance objectivity with empathy., • The role will be filled by a peer (a person with lived experience), • Valid NYS Driver’s License required and a driving record that will permit the use of an Agency vehicle to transport clients to appointments or other activities as needed. Salary Range: $46,000 - $52,000
