Community Mental Health Nurse (Band 6). Salary: £39,959 - £48,117 per annum. 37.5 hours per week. Location: Taylor Centre, Warrior House, Southend. 6 month Fixed Term Contract/Secondment. We are looking for 3 experienced, enthusiastic and dynamic Care Coordinators to manage a caseload of service users with complex health and social care needs following a recovery focused model. Previous care coordination experience is essential for this post. • Responsible for assessment, planning, implementation and evaluation of evidence-based care, including health promotion for a named service user/group., • Responsible for admission, case conference/reviews, and discharge in collaboration with the multi-disciplinary team., • Coordinate care and assessment under the Care Programme Approach of a designated caseload., • Ensure postholder understands and meets professional responsibilities under Safeguarding Vulnerable Adults and Children legislation., • Ensure the requirements of the Mental Health Act 1983 are observed and implemented., • Promote carer and service user involvement within the service., • Establish therapeutic relationships with service users and implement evidence-based therapeutic interventions with appropriate boundaries in accordance with the Nursing and Midwifery Council’s professional code of conduct., • Ensure carers’ assessments, education and support is delivered in accordance with NSF requirements., • Deputise and provide appropriate support and caseload coverage for colleagues/team members., • 27 days holiday plus bank holidays, rising to 33 days after 10 years of service., • Excellent pension up to 14.5% of pensionable pay., • Staff discounts – Blue Light Card, NHS discount offers, and staff benefits., • Interest‑free season ticket loans for travel., • Work bank, flexible working from day one, and job share opportunities.
