Responsibilities may include prescribing and reviewing medicines, supporting medicines optimisation at admission and discharge, initiating and monitoring treatment plans, and producing discharge prescriptions in accordance with Trust policies and professional standards.\nLead and coordinate the day-to-day delivery of clinical pharmacy services within Acute Medicine, while contributing to service development and improvement. Work collaboratively with the Associate Chief Pharmacist – Clinical Services, clinical teams, and senior managers to ensure the provision of safe, clinically effective, and cost-efficient medicines management.\nDevelop, implement, monitor, and evaluate clinical governance initiatives relating to pharmacy services within Acute Medicine.
