Job Description TransCare Program Coordinator Reports To: Director of Interventions Status: Full-Time, Exempt Location: Albany, NY (On-Site, with some evenings and weekends required) About Us In Our Own Voices, Inc. (IOOV) is a nonprofit organization based in Albany, NY, committed to the health, safety, and empowerment of LGBTQ+ people of color. Through culturally affirming programs, IOOV provides comprehensive services that address health equity, advocacy, and community building. The TransCare Program provides HIV/STI/HCV prevention, education, testing, and linkage-to-care services tailored to the unique needs of Transgender and Gender Non-Conforming/Non-Binary (TGNCNB) communities of color. Position Summary The TransCare Program Coordinator is a key member of IOOV’s LGBTQ+ Health Services team. This role is responsible for implementing evidence-based and homegrown interventions, overseeing HIV/STI/HCV awareness and prevention initiatives, conducting risk assessments, and managing general program operations. The Coordinator will also supervise staff, facilitate outreach, and develop culturally responsive programming to reduce health disparities among TGNCNB communities. Major Responsibilities Program Coordination & Implementation * Coordinate HIV/STI/HCV prevention services for TGNCNB communities, with a focus on TGNC people of color. * Implement, recruit for, and facilitate intervention programs, including Project Phoenix and SexTransations. * Expand testing services to increase the number of TGNC individuals who know their HIV/STI/HCV status. * Provide direct case management, referrals, patient navigation, and linkage to care. Community Engagement & Events * Create and implement social marketing campaigns focused on sexual health and wellness. * Plan and facilitate events such as Transgender Day of Remembrance, the Gender-Affirming Drive, and the Sex Toy Drive. * Conduct both in-person and digital outreach to ensure community visibility and engagement. * Build partnerships and network with local agencies, community organizations, and statewide coalitions addressing LGBTQ+ issues. Counseling, Testing & Reporting * Conduct HIV/HCV risk assessments and provide testing and counseling services. * Submit accurate counseling and testing data to the AIDS Institute and IOOV funders. * Ensure compliance with HIPAA and NYS HIV Confidentiality Laws. * Provide quality assurance checks on AIRS data, testing files, and testing practices. Staff Supervision & Leadership * Supervise and support the Case Manager, Community Engagement Specialist, and Peer Advocate. * Provide training, feedback, and performance evaluations for supervised staff. * Promote a collaborative, trauma-informed, and affirming team environment. Administrative & Compliance * Submit timely monthly reports to IOOV and funders. * Participate in staff meetings, trainings, and professional development opportunities. * Perform other duties as assigned. Qualifications Required * Bachelor’s degree in Public Health, Social Work, Human Services, or related field. * At least 2 years of experience in clinical or community-level HIV interventions, counseling, and psychosocial assessments. * Knowledge of social service systems and resources related to HIV/STI/HCV and TGNC communities. * Demonstrated understanding of LGBTQ+ health, PEP, PrEP, gender-affirming care, and harm reduction. * Strong group facilitation, written, and oral communication skills. * Ability to work flexible hours, including evenings and weekends. * Valid, unrestricted driver’s license and reliable transportation. Preferred * Certified in HIV counseling and testing, or ability to obtain certification within 3 months (training provided). * Spanish-speaking. * Supervisory experience. Physical Demands & Work Environment * Ability to lift up to 50 lbs. unassisted. * Prolonged sitting and computer use. * Close visual acuity for detailed work on screens. * Frequent interaction with clients, colleagues, and partners by phone, email, and in person. * Primarily office-based with some community-based outreach.
