I worked in a few different settings throughout my career, hospital outpatient, acute Care, High Volume Outpatient. In each of them I found that I was seeing too many patients per day, wasn't able to provide the care that I wanted to give each patient, and wasn't making the impact that I dreamed of when becoming a physical therapist. I remember coming home late, and being so frustrated. Almost to the point where I was wondering if PT was really the career for me. Don't get me wrong I learned a ton during those work experiences, but I wanted to be different and make a difference in people's lives. So, I started my own little "Side Hustle", while working all my other jobs. About 12 years ago, I decided to open my own practice with the goal of providing one-on-one care to my patients and really getting to know them and their goals. I wanted to help my patients get back to the activities they love without pain. Since opening our doors, we have been fortunate enough to grow quickly and have amazing team members. We are now looking for another new full-time physical therapist to join our team. Over the past couple years we have been able to help 1000s of local residents. SUMMARY • Provision of professional therapy services in compliance with the state Practice Act, any applicable licensure, certification requirement, applicable healthcare standards, governmental laws/regulations, Company policies/procedures, and philosophy of care. The Physical Therapist provides skilled therapy interventions in the treatment of the patient/client’s illness, rehabilitative needs, and preventative care. He/she provides ongoing assessments, problem identification and specific therapeutic interventions, teaching, and training activities. The Physical Therapist provides services in accordance with accepted standards of clinical practice, to facilitate problem resolution, and thereby, the achievement of individual patient/client goals and outcomes. Plan of Treatment • Confers with the patient/client’s physician in developing, implementing, and revising the therapy Plan of Treatment, based on initial and ongoing assessments, and as required by policy/regulation. Ensures proactive revision or addition to the plan of care to meet the patient/client needs as assessed or diagnosed, or as required by policy/regulation., • Initiates and responds appropriately to contact with/from the patient/client, physician, Clinical Care Manager, other members of the healthcare team, in a timely manner as regard patient/client/family needs and status changes., • Acts as a liaison between the patient/client, physician, Clinical Care Manager, other members of the healthcare team and the community in assuring effective care coordination, appropriate resource referrals, and supervision of paraprofessional staff. Provides paraprofessional staff with a personal care plan that reflects the current plan of care, as it relates to therapy.
