The Preventive Services Department provides child welfare services using Evidence Based models to ensure that families are able to provide safe environments for their children, keep children safe in their homes and reduce the need for foster care. Prevention is an essential part of the child welfare continuum and requires a commitment to the wellbeing of all members of a client family. All services are provided in a family’s home and community. POSITION OVERVIEW: The Therapist works with families to assess their needs, develop intervention plans, and deliver therapy sessions that are tailored to the family's unique circumstances. Ensures proactive revision or addition to the plan of care to meet the patient/client needs as assessed or diagnosed, or as required by policy/regulation., • Initiates and responds appropriately to contact with/from the patient/client, physician, Clinical Care Manager, other members of the healthcare team, in a timely manner as regard patient/client/family needs and status changes., • Acts as a liaison between the patient/client, physician, Clinical Care Manager, other members of the healthcare team and the community in assuring effective care coordination, appropriate resource referrals, and supervision of paraprofessional staff. Provides paraprofessional staff with a personal care plan that reflects the current plan of care, as it relates to therapy. Supervises and evaluates the performance of paraprofessional staff, related to physical therapy directives., • The Physical Therapist will prepare, submit, and maintain documentation as required by the Company., • Accurately and thoroughly documents patient care observations, interventions and evaluations on day services are rendered., • Assures that interim (verbal) orders obtained from the physician are accurately transcribed for implementation and/or that Clinical Care Manager is contacted with information to obtain physician’s order. The New Jewish Home has long experience delivering rehabilitation and home care services. The CHHA is currently seeking a part time , 20 hours per week, Physical Therapist to join their team! Job Summary: Responsible for providing and coordinating multidisciplinary health services for patients in the community. Performs the role of Physical Therapist and works with the physician and other team members in the implementation of plan of care. Duties and Responsibilities: • Collaborates with physicians, patients and their families, nurses, therapists, social workers and other members of the patient care team to develop and implement patient specific care and treatment plans., • Utilizes varied solutions to educate and promote the health and independence of each patient., • Assess, monitor, document and report progress of patients’ health and condition using required documents, reports and established timelines.  Provides supervision and evaluation of aides and physical therapy assistants., • Observes signs and symptoms of patients, reports to the physician and/or other appropriate staff as often as needed or as change in the patients’ condition requires., • Completes OASIS assessments timely, accurately and according to OASIS user’s manual definitions. Completes all documentation, patient health status reports as required and forwards appropriately., • Supervises occupational therapy assistant according to state regulations and agency policy., • Assists with the coordination of the total plan of care and maintains continuity of patient care by liaising with other appropriate staff assigned to the same patients. Participates in team/case conferences. Initiates team/care conferences for complex and/or multi, • disciplinary patients when required to ensure coordination of care., • . Ability to communicate effectively both orally and in writing., • Must possess a current, unencumbered license to practice as Physical Therapist in the State of New York., • Successful completion of a Physical Therapy program at a school of occupational therapy approved by the American Physical Therapy Association or the Council on Medical Education and Hospitals of American Medical Association or the committee on Allied Health Education and Accreditation of the American Medical Association., • Master’s degree or Doctorate degree in Physical Therapy., • One year of Physical Therapy experience preferred.
