Duties • Conduct comprehensive evaluations of patients to assess their physical abilities and limitations., • Develop personalized treatment plans based on patient assessments and goals., • Implement physiotherapy interventions, including but not limited to trigger point therapy, orthopedics, and geriatrics., • Provide education to patients and their families regarding treatment options and home exercises., • Collaborate with interdisciplinary teams to ensure cohesive care delivery., • Monitor patient progress and adjust treatment plans as necessary., • Maintain accurate documentation of patient care using EMR systems., • Participate in discharge planning to facilitate smooth transitions from acute care settings., • Knowledge of anatomy and physiology relevant to physical therapy practices., • Familiarity with home & community care practices., • Background in Workers Compensation and Personal Injury outpatient clinic experience is preferred. but not necessary, will train., • Proficiency in medical terminology related to physical therapy., • Previous experience with care plans and discharge planning is highly desirable.
