Position Summary The Leonard Covello Older Adult Program in East Harlem is a vibrant community hub serving adults 60+ through daily activities such as nutritious meals, exercise, arts programming, trips, and educational workshops. The Case Worker provides direct support services that help participants access benefits, housing advocacy, healthcare navigation, and community resources to enhance quality of life at both the Covello and Lehman Village satellite centers. Essential Duties & Responsibilities Caseload Management & Direct Support • Maintain an active caseload in accordance with NYC Aging requirements, providing assistance through in-person meetings, phone calls, and onsite interactions., • Conduct comprehensive assessments and develop individualized care plans in collaboration with clients, families, caregivers, and other professionals., • Assist clients with applications for government benefit programs (e.g., SNAP, Medicaid, Medicare, SCRIE/DRIE)., • Advocate for clients on housing-related matters and tenant concerns., • Maintain timely and accurate client records, case notes, referrals, and assessments in both digital and paper formats., • Ensure compliance with NYC Aging reporting requirements, deadlines, and audit preparation., • Participate in staff meetings, agency events, in-service trainings, and conferences as assigned., • Work closely with the Social Service Unit Supervisor to ensure files and records are organized and accessible., • Assess clients for emergency and stipendiary grant eligibility., • Perform other responsibilities as needed to meet program and client needs. Qualifications Bachelor’s degree in Social Work, Human Services, Psychology, or Gerontology; OR Bachelor’s in another field with one or more years of casework experience in a community social service or social action program. Bilingual Spanish required. Supervisory Responsibilities This position does not have supervisory responsibilities. Work Environment and Physical Demands The Case Worker position is based primarily at the Leonard Covello Older Adult Center, a five-story facility in East Harlem with elevator access, and also provides services at the Lehman Village Older Adult Center. Work is performed in a professional office environment within these centers and involves regular use of standard office equipment, including computers, phones, photocopiers, and filing cabinets. The position requires the ability to remain in a stationary position for extended periods, move throughout the facilities as needed, and communicate effectively with clients, colleagues, and community partners. Occasional travel between sites, offsite meetings, and visits to clients’ homes may be required. Position Type and Hours of Work This is a full-time position, Monday–Friday, 9:00 AM to 5:00 PM. Occasional evening or weekend hours may be required based on program needs and will be scheduled in advance. Compensation & Benefits The Case Worker role offers a competitive salary range between $24.72- $26.92/hr, The annualized equivalent is $45,000 to $49,000 per year. In full compliance with the NYC Pay Transparency Act, with final placement based on experience, skills, and internal equity.
