PROGRAM BACKGROUND: MercyFirst’s A Safe Way Forward Program (SWF) provides child welfare involved families affected by intimate partner violence (IPV) with comprehensive prevention services including therapeutic case management and evidence-based intervention modalities to address IPV. This joint prevention-clinical services model offers comprehensive trauma-informed services to the whole family, including the survivor of IPV, the person causing harm (PCH), and to children exposed to the IPV in the home. Range of services includes in-home visitation, crisis intervention, case management, advocacy, individual, group and family counseling, psycho-education and support, and referral to other appropriate services. The SWF program will serve a total of 65 families residing in Brooklyn. POSITION SUMMARY: The Therapeutic Case Manager is responsible for screenings and assessment, engagement, safety planning and coordination of services and treatment for members of the family system experiencing IPV. Approach to service is family-centered, trauma-informed, strengths-based and emphasizes both short-term and long-term goals to restore and maintain Safety for all family members, Protect children from future exposure to IPV, and promote Healing and Improved Family Relationships. The qualified candidate will be responsible for adherence to reporting requirements and timelines and meeting programmatic outcome measures. REQUIRED QUALIFICATIONS: • Masters Degree in Social Work (MSW) or in a related Human Services field Required, • Prior experience working with children and families known to the Child Welfare system, impacted by Abuse and Trauma Required, • Prior experience working with families affected by IPV Preferred, • Demonstrate cultural competence and a commitment to anti-racism work, • Strong Engagement, Organizational and Communication skills, • Knowledge of and commitment to child welfare best practice standards, • Understanding and willingness to commit to the agency’s mission, core values and Sanctuary commitments, • Ability to accommodate a flexible work schedule, including evenings and weekends and to be on call for emergencies, • Proficiency in Microsoft Office suite, • Understanding and willingness to commit to the agency’s Mission, core values, Sanctuary Commitments and Diversity, Equity, Inclusion, and Belonging (DEIB). PROGRAM BACKGROUND: The Behavior Health Department provides mental health services to children aged 3 to 21 that have been placed in MercyFirst care. Children placed in care are more likely than not to be exposed to adverse childhood experiences and traumatic stress, the Behavior Health Department takes a strength-based, trauma-informed child centered approach to service delivery. SUMMARY: The Psychiatric Nurse Practitioner shall serve as a member of the clinical staff and provide psychiatric input to the treatment planning team. Prescribe and monitor children’s medications and educate families of benefits, risks, and side effects. REQUIRED QUALIFICATIONS: • Graduate of an accredited Psychiatric Nurse Practitioner Program, • Graduate of an accredited master’s degree Program as a Nurse Practitioner with current New York State licensure, • 2 years clinical experience in pediatrics., • Knowledge of or experience in foster care or similar pediatric populations., • Valid DEA license., • Must have and maintain a valid driver’s license with satisfactory driving record. POSITION SUMMARY: The Birth Parent Advocate will provide advocacy, support, and referral services for birth parents whose children are in foster care. The Parent Advocate helps the parent understand and navigate the foster care system, make decisions for their families, remain hopeful, and address family challenges so that they can accomplish their goal of reunification. The focus of this job is building connections with parents and working collaboratively with case planners. Each Parent Advocate will be assigned to a case planning unit and will support parents and bring their experience to the foster care agency decision-making process so that children safely return home. Parent Advocates with their own personal prior lived experience of the child welfare system will be effective in informing and supporting parents with children in foster care and help to improve outcomes for children and families. POSITION SUMMARY: The Aftercare worker for the QRTP program will be responsible for assessment, engagement and case management services for clients and families towards successful transition out of QRTP placement. This qualified candidate will be a part of a multi-disciplinary team and work alongside the QRTP caseplanner and will refer guardian for supportive services, including medical, clinical and concrete services. Upon the youth’ transition out of the QRTP program, the Aftercare Caseworker will provide the youth and their family with continued casework support up until the youth's placement order expires, to ensure successful reintegration into the community and in the home. Case planner will present the program’s efforts in legal proceedings, ACS/OCFS meetings and other interagency conferences. The qualified candidate will complete the required progress notes, assessments, Psychosocial reports, FASPs, court reports, etc within timely mandates. RESPONSIBILITIES: • Conduct regular casework sessions with each client in accordance with Federal, State, and Local mandates., • While youth is in placement: minimal monthly meeting with youth and family., • Meet with youth 2x weekly/legal guardian weekly for the first 6 weeks; once weekly thereafter. Home assessments within the first week of youth’s placement, monthly thereafter., • Collateral contact weekly with youth's school/therapist/other resources., • Document all contacts in designated platforms within established timeframes, • Prepare all required documentation on each case which include progress notes, psychosocial reports, permanency hearing notes, FASPs, etc., • Attend legal proceedings such as family court hearings, • Actively participate in staff training and supervision meetings., • Respond to emergencies as needed and/or directed, • Participate in Family Team Conference, Child Safety conference and other organized conferences., • Availability to assist with after hour supports to clients and families., • Provide after-care services to clients upon discharge from residential care., • Participation in weekly supervision, program meetings, trainings and workshops to enhance professional development and advance program development initiatives, • Establish and sustain a workplace environment that is respectful and supportive of staff, all members of family, community service providers, and other external stakeholders, • Collaborate with other community-based service providers to advance client-specific needs and assist with obtaining services including, but not limited to housing, financial assistance, health care, mental health services, education services, legal advocacy, and parenting support. POSITION SUMMARY: The Housing Specialist is responsible for working with birth parents and youth in our Family Foster Care and Residential programs. The Housing Specialist will help our clients secure and obtain a permanent place to live. The ideal candidate will have experience working with at risk youth and have a strong knowledge base of the following: NYCHA, Supportive Housing, Section 8, and other related programs.
