Job Description JOB SUMMARY: The Care Coordinator functions as a member of an interdisciplinary team to provide care coordination to a caseload of severely mentally ill adults with multiple medical comorbidities and/or co-occurring substance abuse disorders and/or medically ill individuals. Advocates for and supports the client, engages with community agencies/health care providers and others on his behalf to ensure access to services needed to increase wellness self-management and reduce emergency room visits and/ or hospitalizations. Provides clinical support to the Team by providing consultation, education, information around psychosocial and/or substance abuse conditions, interventions, resources to maintain focus on outcomes and best practices. Reasonable accommodations may be made to enable qualified individuals with a disability to perform the essential duties unless this causes undue hardship to the agency.) • Conducts initial and ongoing assessments of assigned clients to document strengths, needs, goals, and resources., • Participates in the development/documentation /review and update of client centered comprehensive integrated, interdisciplinary care plan in consultation with other team members to ensure focus on desired outcomes., • Maintains effective communications with clients, primary care physicians, substance abuse, and mental healthcare providers, family, collateral resources and other Agency staff on behalf of clients., • Maintains documents, records, statistics, and other related reports in an organized, timely, and accurate manner as per policy and procedure., • Coordinates care planning with other providers of services/ resources to ensure goal directed, collaborative care, including care transitions., • Works as part of a Care Coordination team; attends and participates in team meetings to provide input/feedback around psychosocial and medical conditions conditions/comorbidities to review client status, update plans and goals, review outcomes to further program goals., • Acts as a resources/consultant to all team members on psychosocial, medical and/or substance abuse issues and resources., • Provides telephonic as well as face-to-face outreach, engagement, and service planning in the field., • Acts as a linkage to community services including medical, behavioral, residential, entitlement and any other needed services per interdisciplinary care plan., • Monitors overall service delivery to clients to ensure coordination and continuity; advocates with service providers/resources as needed., • Provides crisis intervention and follow-up., • May be assigned other tasks and duties reasonably related to the job responsibilities., • Working knowledge of computer software and electronic health record systems, • Demonstrated competency in written, verbal, and computational skills to present and document records in accordance with program standards., • Experienced in and demonstrated comprehensive understanding and working knowledge of the interdisciplinary planning process and the developmental treatment model., • Knowledge of Medicaid, Social Security and other entitlements preferred., • Excellent interpersonal skills required., • Specific training for the designated assessment tool(s), the array of services and supports available, and the client-centered service planning process. POSITION SUMMARY: CHASI’s Next Step Resource and Recovery Center is a hub for people and families struggling with drug and alcohol addiction and substance use. The Next Step Center is a caring, energetic recovery support community for Staten Islanders in all stages of recovery from dependency on drugs or alcohol. The Housing Advocate is responsible for the provision of services and referrals on behalf of clients with opioid use and polysubstance use disorders to ensure the stabilization of their current housing conditions, facilitate expeditious placement for those in need of permanent housing, and assist clients in becoming self-sufficient. In this dual role, the Housing Advocate will also serve as a Social Care Referral Coordinator, accepting and managing referrals for housing and tenancy services. This includes providing case management support to address rental and utilities arrears, coordinating home modification and remediation needs, and connecting clients to appropriate social care resources that promote housing stability and overall well-being. Key responsibilities include conducting housing retention intakes and assessments; developing individualized housing stabilization plans; providing and coordinating direct services in alignment with those plans; and maximizing housing tenure. The Housing Advocate must be knowledgeable about the full range of housing options available to individuals with substance use or co-occurring disorders. The role also requires building and maintaining strong relationships with housing providers, prospective landlords, brokers, agents, and property management companies. In addition, the Housing Advocate will collaborate with community partners to develop and maintain a comprehensive housing resource manual with services available on Staten Island and throughout the other boroughs. DUTIES & RESPONSIBILITIES: • Conduct initial and ongoing housing needs assessments with referred clients, members of the Next Step Resource and Recovery Center, Peer Training Institute, and their families., • Initiate and complete the process to secure decent, affordable permanent housing, including preparation and submission of the 2010E housing application., • Serve as the Social Care Referral Coordinator, receiving and managing referrals for Housing and Tenancy Services through the CHASI Social Care Network, including case management for rental and utility arrears, home modification, remediation, and other tenancy supports., • Assist clients in building the skills and supports needed to maintain their households and finances, perform activities of daily living independently, uphold the terms of their leases, and prevent avoidable evictions., • Intervene and provide crisis mitigation strategies to maximize tenant safety and security., • Assess and promote ongoing linkage to medical, behavioral health, and community support services, including intensive case management for those with multiple needs. Communicate and coordinate housing-related activities with referring program staff to ensure continuity of care and service connection., • Research, identify, and catalog current housing options locally and across boroughs for individuals in recovery, including those with specialized needs such as mental health support, family housing, or safety-related accommodations., • Develop and maintain a Housing Readiness Assessment and individualized housing stabilization plans., • Establish and maintain relationships with housing providers, landlords, brokers, agents, and property management companies to expand housing opportunities., • Communicate, engage, and advocate on behalf of clients to support housing stability and self-sufficiency., • Prepare timely, accurate weekly and monthly data for program and funder reporting requirements., • Participate actively in team meetings, supervision, and case conferences., • Collaborate with community partners to contribute to a comprehensive housing resource manual for Staten Island and the surrounding boroughs., • Perform all functions in alignment with CHASI’s Mission, Vision, and Core Values., • Other duties as assigned. Job Description TITLE: Medication Assisted Treatment (MAT) Coordinator DEPARTMENT: Clinical FLSA CODE: Non-exempt PROGRAM: The Annex - 13 Hell Gate Circle, Ward's Island, NY 10035 MAJOR FUNCTIONS: The Medication Assisted Treatment (MAT) Coordinator will be responsible for overseeing the delivery of medication assisted treatment services to clients with a Substance Abuse Disorder (SUD). They work closely with the multidisciplinary team to ensure that clients receive the appropriate support services needed to achieve and maintain recovery. Additionally, the MAT Coordinator acts as the point of contact between Odyssey House and partner Opioid Treatment Programs (OTP’s) from which Odyssey House clients receive services. SPECIFIC DUTIES & RESPONSIBILITIES: • Coordinate the delivery of medication assisted treatment services to clients in Odyssey House’s SUD programs., • Collaborate with internal and external treatment professionals to develop and implement treatment plans., • Provide education and support to clients on medication assisted treatment options., • Maintain a caseload of clients prescribed or interested in medication assisted treatment., • Facilitate a weekly Opioid Use Disorder (OUD) group/seminar for client’s diagnosis with OUD., • Monitor client progress and adjust treatment plan accordingly., • Maintain accurate client records and documentation., • Ensure compliance with client records in accordance with State, Federal and HIPPA regulations, • Escort clients to and from the Opioid Treatment Program (OTP), while adhering to Odyssey House’s OTP protocol., • Communicate with OTP in accordance with Odyssey House’s OTP Protocol., • Provide case management and care coordination services specific to medication assisted treatment for all clients assigned to their caseload., • Ensure compliance with all laws, regulations and policies related to medication assisted treatment., • Assists with monitoring toxicology specimens for clients and external referral sources., • Establishes and maintains a positive and cooperative working relationship with outside agencies., • Performs other related duties as assigned by Clinical Director and other management as requiredREQUIREMENTS: EDUCATION/TRAINING AND EXPERIENCE KNOWLEDGE, SKILLS AND ABILITIES, • Minimum of (2) years of experience working in substance use disorder treatment or related field., • Bachelor’s degree in social work, psychology, or a related field preferred., • CASAC or CASAC-T credential, • Proficiency with computer operation (Microsoft Word, Excel, AWARDS, and Outlook programs)., • Knowledge of medication assisted treatment options and evidence-based practices., • Ability to work collaboratively with a team of healthcare and substance use disorder providers., • Ability to deliver service in a way that is responsive, respectful, caring and helpful while maintaining confidentiality., • Ability to establish and maintain a positive and cooperative working relationship with all employees., • Comfortable working in a fast-paced environment and managing multiple tasks simultaneously. The Medication Assisted Treatment Coordinator will work in an Odyssey House Substance Use Disorder program. They may be required to work evenings, weekends, and holidays to accommodate client needs. The position may require travel to different Odyssey House treatment sites. PHYSICAL REQUIREMENTS The Medication Assisted Treatment Coordinator may be required to stand or sit for extended periods of time and perform tasks that require manual dexterity. They may also be required to lift or move objects up to 20 pounds.
