Case management focusses on removing barriers to access needed services in an effort to reduce and/or avoid recidivism. Ensure that all clients’ basic needs are met and that clients are connected to needed services, such as mental health, substance use, benefits, legal, etc. The operations component focuses on the housing units meeting habitability standards by working with landlords and management companies to address major repair issues and complete minor repairs, as well as identifying new units, negotiating leases and making units ready when there is a vacancy. Position: Program Supervisor Reports To: Program Manager Location: 19 Winthrop Street, Brooklyn NY 11225 What The Program Supervisor Does: • Perform Quality assurance field and home visits to ensure client satisfaction; accompany Case Managers and Housing Specialist at home visit monthly to examine interaction and quality services., • Supervision of staff, collaboration with Program Manager to complete performance appraisals, coaching, counseling, mentoring staff., • Troubleshoot customer/client issues & concerns, and make decisions in accordance with program policies, procedures, and protocols., • Read progress notes in the absence of the Program Manager and ensure staff are following the progress note template., • Work with direct reports to review and improve any customer feedback, recommend training, and other appropriate recommendations., • Lead appropriately any pushback or resistance to change from customers., • Monitor customers' progress towards their goals weekly and ensure Case Managers are developing and addressing realistic Service Plan goals, e.g medical, mental health, education etc., • Review all documentation related to customers' progress for accuracy, completeness, and clarity., • .Participate in administrative and staff meetings as requested., • Ensure staff are updating TMS Maven monthly and ensure all data reflects Service Plan progress., • Prepare marketing materials for the program., • As necessary maintain caseload., • Plan, coordinate and facilitate social/peer support events, including group facilitation for clients and the quarterly community advisory board community meetings., • Two years of applicable experience and/or equivalent experience., • One year of experience supervising others., • Knowledge about, understanding of, and ability to work closely with people who are homeless, those with a history of drug use and/or mentally ill. Certified Recovery Peer Advocate services are peer-delivered services with a rehabilitation and recovery focus. The Certified Recovery Peer Advocate uses trauma-informed, non-clinical assistance to achieve long-term recovery from a behavioral health disorder. The Clinical Supervisor is responsible for adherence to all applicable licensing, supervision, billing, documentation requirements and overseeing the timeliness and quality of a staff member’s clinical work. The Clinical Supervisor is responsible for client case record reviews, performance improvement and quality improvement to include all required monthly reports. KEY ESSENTIAL FUNCTIONS • Responsible for the daily operations and performance of the OTP program, • Manage OTP clinic operations to budgeted and planned results., • Work closely with staff via weekly supervision to ensure the completion of performance goals. Provides ongoing, regular feedback and performance evaluation to clinicians on the quality and timeliness of their clinical work as well as their caseload. Also solicits feedback from clinicians regarding supervision and whether needs are being met., • Monitor caseloads and ensure the supervisees comply with all agency, clinical, legal and ethical practices. Review and co-sign supervisees’ clinical documentation to include but not limited to methadone dose change request forms, bio-psychosocial history, initial treatment plans and treatment plans reviews., • Becomes familiar with APG and ICD-10 billing codes to include DSM V diagnosis codes., • Oversight and monitoring of billable hours of service required. Provide support, training, and remediation where needed to assist supervisees in meeting program’s billing expectations and caseload requirements, charting, recordkeeping, documentation standards and all other quality assurance standards., • Communicate any concerns about quality of clinician’s work to Program Director in a timely manner., • Become familiar with ACACIA policies and procedures and implements in OTP program. Ensures all administrative and clinical policies and procedures are implemented appropriately and effectively. General Description: The Clinical Director oversees the operation of the Clinical Inpatient and Outpatient Programs. Job Description About Us: We are a virtual-first primary care and mental health medical practice. We are not your granddad's mental health practice. We focus on two services: 1.)
