Job Description Team Supervisor- Adult Care Coordination Program For over 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens, and currently offers 160-plus programs and services for children, youth, adults, seniors, and those struggling with mental illness. Catholic Charities provides individuals with serious mental illness, complex medical needs and substance abuse issues with comprehensive care coordination and case management services. Our Care Coordination team works with the New York State Health Homes Program to help adults meet their health and wellness goals, and live self-directed lives within their community. RESPONSIBILITIES: The Health Home Team Supervisor oversees a team of direct care staff charged with coordinating all aspects of care for assigned clients. Clients present with mental health needs, complex medical conditions, co-occurring substance use disorders, and a wide array of social service needs. In addition to overseeing client-centered care, the Team Supervisor has additional goals of reducing avoidable health care costs, specifically preventable hospital admissions/readmissions and avoidable emergency room visits, and improving client’s health outcomes by addressing primary medical, specialist and behavioral health care through contractual arrangements with appropriate service providers. The Team Supervisor is a driving force behind comprehensive, integrated services, ensuring direct care staff conduct a comprehensive assessment of clients and facilitate access to the full range of medical, behavioral health, substance use, social and psychosocial services in the community, in a timely, efficient, and effective manner. Clients are provided care coordination services in their home and in the community. The program applies a person-centered and recovery-oriented approach to care, empowering the individual to be active participants in their healthcare journey. The Team Supervisor also maintains health home networks with primary medical and specialty practitioners and mental health providers, substance abuse service providers, community-based organizations, managed care plans, emergency rooms, hospitals, and residential/rehabilitation settings to ensure coordinated, and safe transitions of in care for clients who require transfer to/from various levels of care. The Team Supervisor monitors daily activities of staff, ensuring the scope, frequency and duration of client encounters are adequate to meet client needs, and substantiate monthly billing of core services. This includes oversight of progress notes documentation, assessments, Care Plans, and billing tools through regular chart reviews and staff supervisions. The Team Supervisor also ensures implementation of a continuous quality improvement plan for the team and conducts workshops and trainings as needed. This position is currently per diem simply meaning you will be paid per service provided at your agreed pay rate. Responsibilities • Assist in conducting client assessments to determine appropriate level of care and specific treatment needs., • Co-facilitate group therapy sessions and provide support in individual counseling under the supervision of licensed staff., • Participate in the development and implementation of treatment plans., • Assist with case management duties, including client follow-ups and coordinating care with other service providers., • Maintain accurate and confidential client records in compliance with OASAS regulations., • Participate in team meetings and training sessions to enhance skills and knowledge in substance abuse treatment., • CASAC-T or greater, LMSW, MHC-LP, LMHC, LCSW, • 1 year of experience in substance abuse or mental health counseling, • Strong interest in substance abuse counseling and mental health services., • Excellent verbal and written communication skills., • Ability to maintain professional boundaries and confidentiality., • Work in a supportive and mission-driven environment dedicated to excellence., • Make a tangible difference in the lives of individuals and families in your community. Job Description SUMMARY This position is primarily responsible for the provision of clinical counseling services and comprehensive case management. This position reports directly to the Director of Outpatient Services. ESSENTIAL DUTIES AND RESPONSIBILITIES: Core duties and responsibilities include the following. Other duties may be assigned. • Collects and analyzes assessment data which includes substance use, mental health and co-occurring disorders., • Completes intake process and arrives at treatment recommendations and level of care decisions that are appropriate to client’s level of functioning and medical necessity., • Completes psychosocial assessments., • Develops initial and comprehensive individualized treatment plans based on client’s needs, using client’s words, and that have measurable objectives, within established timelines, based on assessment, psychosocial, client’s assessed stage of change, and in consultation with client and treatment team as appropriate., • Provides individual counseling based on treatment plan and client centered needs., • Conducts group counseling. To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Job Description Sign-on eligible FUNCTION: To provide comprehensive clinical case management services that include: assessments; individual, group and family treatment; treatment plan coordination; referent relations; discharge planning; and intervention with client support systems, including social service programs and managed care. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties as required. Provides/ensures comprehensive evaluation of patient's functioning as defined by facility policy and procedures. Coordinates the development and implementation of the multidisciplinary treatment plan; coordinates treatment plan review; performs these activities within established time frames. Documents patient progress and own interventions in case record. Need to remain within time frames of regulatory requirements. Provides individual, group and family treatment (psycho-education, life skills) as identified in patient treatment plans and requested by program director. Performs utilization review, provides progress reports and develop aftercare plans in conjunction with referring persons/agencies, EAPs and managed care/insurance entities. Attend and/or facilitate Family program when requested by supervisor. Participates in facility program and quality improvement activities. Participates in quality improvement activities related to position and as requested by program director. Responds to emergency situations in the facility by following appropriate procedures for ensuring patient safety during emergency procedures. Supervises interns and volunteers. Is actively involved in milieu management through attendance at community meetings, supervision of patients during activities and interventions with patients found in violation of program/ facility rules. Participates in Performance Improvement Initiatives. Maintains professional competence in discipline and treatment specialty areas through continuing education and supervision. High school diploma, general education degree (GED) or bachelor’s degree with CASAC certification or LMSW, LCSW, LMHC. Background checks, pre-employment & drug screenings required.
