Job Description Nursing Personnel Home Care is seeking a highly organized, motivated, and detail oriented Care Coordination Specialist to join our growing team. This is a fast paced operational role focused on coordinating home care services, scheduling field staff, and ensuring patients receive exceptional support and continuity of care. The ideal candidate is someone who thrives in a high energy environment, can multitask efficiently, communicates professionally, and is able to problem solve quickly under pressure. Responsibilities • Answer incoming phone calls and direct inquiries appropriately, • Greet visitors and provide professional customer service, • Perform data entry and maintain accurate patient and employee records, • Scan, upload, file, and organize documentation within electronic systems, • Assist with scheduling appointments, assessments, and patient services, • Verify the completeness and accuracy of forms and supporting documentation, • Communicate with patients, caregivers, referral sources, and staff regarding administrative matters, • Monitor and process incoming faxes, emails, and correspondence, • Assist various departments with clerical and administrative projects, • Maintain confidentiality of patient and employee information in accordance with HIPAA regulations, • Perform other administrative duties as assigned Qualifications: • Prior scheduling, coordination, customer service, healthcare, or home care experience preferred, but not required, • Must be bilingual in English and Spanish, • Strong communication and organizational skills, • Ability to multitask and work in a fast paced environment, • Comfortable handling high call volume and time sensitive situations, • Strong computer skills and ability to learn internal systems quickly, • Experience in home care or healthcare operations is a plus What We Offer: • Competitive compensation, • Growth opportunities within a rapidly expanding healthcare organization, • Supportive team environment, • Stable, long term career opportunity, • Opportunity to make a direct impact on patient care and operations Nursing Personnel Home Care is an Equal Opportunity Employer.
