Registered Nurse (RN) – Full Time HIV Medicine Clinic - Albany, NY We have an exciting part time opportunity for a caring and compassionate RN to join our HIV Medicine Clinic team of dedicated healthcare providers! The Albany Medical College Division of HIV Medicine (DHIVM) offers comprehensive HIV services including primary medical care, medical case management, medication adherence, peer services, substance abuse counseling, mental health counseling and psychiatric services, nutrition services, gynecological services, Hepatitis B and Hepatitis C management/ treatment, referral to clinical trials, pre-exposure prophylaxis (PrEP), post exposure prophylaxis (PEP), and HIV counseling and testing. This is a dynamic, quality focused, team-based department. Under the direction of the practice physician and/or advanced practice provider (APP) and the supervision of the Nurse Manager (RN) and/or Nurse Supervisor (RN), the RN may provide direct patient care, patient triage (in-person and telephonic), assessment, planning, directing and evaluating of a patient’s specific care plan and clinic visit. Able to work effectively within a team of multi-specialty professionals. Job Description Bilingual Case Manager of Functional Family Therapy ESSENTIAL DUTIES AND RESPONSIBILITIES RiseBoro’s Functional Family Therapy- Adult program is a therapeutic model that operates within its Clinical Services unit and is a strength-based, evidence-informed, culturally responsive intervention program that, at its core, aims to reduce community and interpersonal violence by engaging people in a change process that nurtures hope and strengthens relationships. Specific attention is paid to interfamilial and extra-familial factors, and how they present within and influence therapeutic processes. The SWF program will serve a total of 65 families residing in Brooklyn. Job Description The Unaccompanied Children Program provides long term and transitional residential care and other supportive services to Unaccompanied Children (UC) who are under legal custody of the Federal Government. Working in collaboration with the federal Office of Refugee Resettlement, ORR, our program provides a safe and nurturing environment for youth in a Basic Shelter program located at our licensed residential facilities as well as a Transitional Foster Care program in foster homes in the Bronx, Brooklyn, Queens and Westchester. Range of services includes in-home visitation, crisis intervention, case management, advocacy, individual, group and family counseling, psycho-education and support, and referral to other appropriate services. The SWF program will serve a total of 65 families residing in Brooklyn. POSITION SUMMARY: The Therapeutic Case Manager is responsible for screenings and assessment, engagement, safety planning and coordination of services and treatment for members of the family system experiencing IPV. Approach to service is family-centered, trauma-informed, strengths-based and emphasizes both short-term and long-term goals to restore and maintain Safety for all family members, Protect children from future exposure to IPV, and promote Healing and Improved Family Relationships. The overall goal of the program is to reduce the impact of victimization through advocacy, counseling and other support services.
