Job Description Summary: The NYS 1115 Program Navigator plays a key role in helping New Yorkers understand, access, and utilize services available through New York State's 1115 Medicaid Waiver program. This position provides outreach, education, enrollment assistance, care coordination support, and linkage to community-based and clinical services that address health-related social needs (HRSN), improve health outcomes, and promote health equity. The Intensive Case Manager will navigate clients through the many complex systems called into action in service plans, such as behavioral health services, medical services, criminal justice, family court, and child protective services. The Intensive Case Manager will be a strong advocate who can provide ongoing support to clients with the police, prosecutors, housing, shelters, schools, public assistance, and other service providers.
