Licensed Practical Nurse, Correctional Health Services*Evening Tour, 4p-12a, Various Locations Schedule: Sunday, Monday, Wednesday, Thursday, Saturday: 4 pm- 12 am Employed by PAGNY for The Division of Correctional Health Services (CHS) of New York City Health + Hospitals (H+H) is responsible for the delivery of medical care, mental health care, and discharge planning in the jail system. Approximately 25% of new admissions per year to the NYC jail system will receive care on the mental health service. This service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, a strong DBT training program, clinics in each of the facilities, substance use treatment, integrated re- entry services, and relationships with multiple training institutions throughout the city. Nurses work in collaboration with the interdisciplinary team to provide patients with care, education, and support. LPNs participate in the planning, implementation, and evaluation of nursing care, based on the objectives, standards, and policies of practical nursing. LPNs work under the direction of a Registered Nurse and/or Physician. Scheduling note: Nurses are required to work every other weekend. Responsibilities: • Maintains a comfortable, orderly, safe, and clean environment for patients., • Measures & records patients’ vital signs (such as height, weight, temperature, blood pressure, pulse, respirations, food/fluid input/output, etc.) in the electronic health record., • Assists with bathing, dressing, and personal hygiene of the patient., • Monitors catheters., • Dresses wounds., • Assists with patient positioning, standing, walking, and transferring., • Collect samples of blood and/or urine for testing., • Performs routine laboratory tests as ordered., • Prepares, administers, and documents prescribed medications via the ordered route., • Reports adverse reactions to medications or treatments according to the policy regarding medication administration., • Assists in relevant patient teaching., • Records all care information concisely, accurately, completely, promptly, in the appropriate format, and on the appropriate forms/templates, • Performs other position-related duties as assigned, depending on the assignment setting, • Direct care provided by unlicensed staff., • Upholds HIPAA regulations., • Floats among various clinical services, where qualified., • Punctual and dependent on assigned/confirmed shifts. EMERGENCY RESPONSE • Responding to/addressing medical emergencies, such as falls, heart attacks, strokes, burns, and recovering post-operative patients., • Provide clinically appropriate care as directed by the Registered Nurse and/or Physician., • When clinically indicated, provide care, such as CPR, AED, prevent shock, control severe bleeding, prevent spinal damage, etc., • Monitor patients' condition and keep it stable while en route to the clinic., • Aid in the preparation of the transfer of patients by Urgicare and/or EMS to the emergency units or hospitals., • Document the medical treatment that is given to the patient., • Keep the equipment clean after use; check and replace damaged and used supplies., • Ability to lift fifty (50) pounds., • Moving, lifting, or transferring patients may involve lifting up to 100 pounds., • Ability to stand for extended periods., • Fine motor skills. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units (“mental observation units”) for individuals with serious mental illness, a 24-hour outpatient-level clinic in each facility, substance use treatment, integrated re-entry services, and relationships with multiple training institutions throughout the city. Each of the 12 facilities in the jail system has unique patient population and environmental characteristics; each Psychiatrist will be matched with the population that most closely meets their clinical expertise and desired work location. Experience working in correctional settings is not required, however the ability to work closely and effectively with multiple other disciplines including security staff is critical. • Provides psychiatric care, including admission evaluations, follow-ups and urgent and emergent consultations to individuals enrolled in the mental health service, • Provide consultation to non-prescribing mental health staff and medical staff about diagnosis, treatment and/or the need for psychiatric medication, • Participate in team meetings with the treatment staff, on a daily basis if working on a mental observation unit and on a weekly basis if working in one of the clinics, • Regularly participate in the creation of individual treatment plans for patients, and review plans already in place, • Evaluate patients for the need for a higher level of care, including hospitalizations. A significant number of individuals in the NYC jail system will receive care on the mental health service. The service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, clinics in each of the facilities, substance use treatment, medication assisted treatment (MAT), integrated re-entry services that includes alternatives to incarceration, and relationships with multiple training institutions throughout the city. Furthermore, CHS has been engaged in an effort to better identify people with neurodevelopmental/neurocognitive disorders and to utilize psychological testing to connect people to services in the community (e.g., with OPWDD). Summary of position The Senior Psychiatrist is the psychiatric leader of the facility, clinical area, or program and works in close collaboration with the Mental Health Unit Chief, Clinical Supervisors, and Supervising Psychiatrists (if applicable) to ensure that safe, holistic and evidence-based therapeutic interventions are provided for the individuals incarcerated in the New York City jail system. North Infirmary Command (NIC), West Facility (WF), and the Contagious Disease Unit (CDU) are the jail facilities that house a significant number of incarcerated individuals with advanced age, complex medical problems, disabilities, and/or enhanced nursing needs. The Senior CL Psychiatrist working in this facility will lead the team in diagnostic review, treatment planning, and interdisciplinary case conferencing with medical, nursing, and social work for individuals housed in this facility with comorbid mental illness, including direct patient care as well as supervision of other psychiatric providers. The Senior CL Psychiatrist will also consult on the care of medically complex patients in other jail facilities. This includes active participation in quality improvement, staff and patient safety initiatives, treatment supervision, and personnel mentorship and management., • In coordination with the facility-specific Mental Health Unit Chief and/or Senior Clinical Supervisor, design, implementation, ongoing review, and monitoring of relevant clinical facility-specific procedures, policies and operating manuals, • Direct clinical individual and group supervision of the relevant psychiatric prescribing staff, including psychiatrists, psychiatric nurse practitioners, and psychiatric physician assistants. The Department is comprised of 70+ healthcare professionals who are responsible for providing re-entry services to individuals detained in the NYC jail system. The Social Work Department supports patients’ rights and criminal-legal advocacy efforts in collaboration with patients’ criminal defense attorneys, Alternative to Incarceration (ATI) programs, and other post-release supervision providers. Additionally, our Social Workers coordinate closely with the New York State Office of Mental Health (OMH) and the NYC Department of Health & Mental Health (DOHMH) to provide robust services to our most vulnerable patients. This position reports to the Associate Director of Social Work Reentry and is responsible for collaborating with the Mental Health Service on assessing patients’ appropriate level of care post-release. Requests from other agencies should be referred to the pharmacist on duty., • Prints and reviews all transfer reports, checks if the patients is on medication, prints patient medication profile and fills needed doses of medication for subsequent checking by pharmacist. If patient is on methadone or DOT medications, tells pharmacist of such., • Transports patient cassettes to medication clinic areas and exchanges same when necessary., • Files completed fill lists, updates lists, distribution lists, methadone sheets and other materials according to procedure., • Checks, rotates and restocks all drug stock areas. Fills Pyxis machines as required. Files Pyxis reports with non pyxis stock records and retains them for 1 week. On a monthly basis checks for outdated medications in Pyxis, clinic floor stock, pharmacy stock and other areas where pharmacy stores medications. For non pyxis floor stock, maintains established par levels based on individual clinic or infirmary need. Records issues of stock on the non pyxis Floor Stock Inventory and Issue Record., • Delivers emergency boxes and other requested items to clinic or infirmary areas as needed., • Prepares weekly pharmacy stock order by utilizing par levels and building usage patterns for review by the pharmacist. Enters order into IDWhere and transmits on both a daily and weekly basis to packaging. Reconciles packing slip or invoice against items received. Job Description Overview Employed by PAGNY for The Division of Correctional Health Services (CHS) of New York City Health + Hospitals (H+H) is responsible for the delivery of medical care, mental health care, and discharge planning in the jail system. Approximately 25% of new admissions per year to the NYC jail system will receive care on the mental health service. This service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, a strong DBT training program, clinics in each of the facilities, substance use treatment, integrated re- entry services, and relationships with multiple training institutions throughout the city. Responsibilities Scheduler: Monday - Friday, 8am - 4pm Under directions of the Director of Operations, with broad latitude for independent judgement and Initiative, schedules staff within the correctional facilities or reassigns staff to ensure that vacancies are Filled. This position is responsible for tracking facility staffing and for relocating staff as needed to meet Demand. In addition, the Staff Scheduler shall be responsible for answering and logging all telephone Calls made to the Operations Office, directing such calls to the appropriate party (s) and paging CHS staff members as needed. • Generates accurate schedules for medical and ancillary staff., • Distributes overtime assignments in an equitable manner., • Ensures that each facility is staffed correctly pursuant to the staffing matrix for each tour and day., • Confirms schedules with each facility and posts the schedules so that all facility staff can view them., • Enforces rules and scheduling policies., • Supports multiple facilities or parallel schedules., • Automatically reassigns staff and/or schedules temporary staff to provide appropriate coverage in the event of employee absences or increase in demand., • Answers phones, maintains sick call log and pages staff as needed., • Provides reports as needed., • Performs other duties as required. Qualifications • An Associate’s degree is required; BA/BS preferred. Responsibilities Under the supervision of a Senior/Supervising Psychiatrist in the Mental Health Service of CHS, this position requires the provision of high quality, team-based psychiatric services including psychopharmacologic interventions, for individuals incarcerated in the New York City jail system. Each of the facilities in the jail system has unique patient population and environmental characteristics; each psychiatric Physician’s Assistant will be matched with the population that most closely meets their clinical expertise and desired work location and schedule. Experience working in correctional settings is not required, however the ability to work closely and effectively with multiple other disciplines, including security staff, is critical. • Provide psychiatric care, including admission evaluations, follow-ups, and urgent and emergent consultations to individuals with mental health needs., • Participate in the creation of an individualized treatment plan after discussing the case with the mental health care team, collecting and reviewing psychosocial data, medical history, nursing assessment, and other pertinent information., • Evaluate patients for the need for a higher level of care, including hospitalization and escalate care when clinically appropriate. If a patient is hospitalized, maintain contact with the hospital psychiatric staff to ensure continuity of care, • Provide patient education, specifically around issues related to medications and treatment adherence, and aftercare services. Document all activities in the electronic medical record in a timely, concise and clear manner., • Integrate nursing and medical concepts in planning care for medically complicated psychiatric patients., • Provide consultation to non-prescribing mental health staff and medical staff about diagnosis, treatment and/or the need for psychiatric medication., • Counsel patients using advanced psychotherapeutic techniques, including in group settings if needed., • Participate in team meetings with the treatment staff, on a daily basis if working on a mental observation unit and on a weekly basis if working in one of the clinics., • Engage in clinical supervision, as indicated, with the Supervising or Senior psychiatrist., • Provide clinical oversight for psychiatric physician’s assistant trainees as needed, including appropriate direct supervision and documentation of evaluation and feedback., • Demonstrate advanced knowledge of psychopathology in assessing patients in need of mental health treatment, including mental status examination and diagnosis.
