ESSENTIAL JOB FUNCTIONS: Through the employee’s own efforts, the employee accomplishes the following essential functions: • Supervisor will have a limited client caseload at the shelter., • Organize and conduct workshops based on client needs at the facility., • Supervisor collaborates to develop programming to address clients’ needs related to housing, substance abuse treatment, mental health and/or employment., • Supervisor provide crisis management and interventions with clients, ensure teamwork and ensure consistent services in accordance with program policies and procedures., • Supervisor responsible for Client Engagement also develop workshops for client engagement, conduct field visits, plan programming to increase community and citywide resources and incentives for client participation., • Responsible for retaining assigned qualified staff and ensures professional growth and development., • Reviews treatment/recovery plans and progress notes, written and electronic, to ensure that clinical services are appropriate with the individual client’s level of care and needs and appropriate with the diagnosis., • Communicates frequently with varied referral sources to maintain working and problem-solving relationships with medical, psychiatric, social services, parole, criminal justice, labor and other resources for client recruitments., • Work from a strength based, family focused, client centered perspective and identify and fortify the strengths that each family has., • Assist families with specific stressors associated with living in a shelter., • Ensure that families are receiving full support in shelter and, when necessary, refer families for counseling or other services., • Aid families in understanding the full range of benefits to which they are entitled and assist with accessing said benefits., • Confer and consult with professional and technical personnel in implementing a multidisciplinary approach to client care and well-being., • Serve as agency/program liaison to community agencies and/or groups., • Provide clear reports that capture key findings to include provider areas of strength, and recommendations for improvement., • Ensure that all case management is documented in database system, Cares., • Attend professional development trainings., • Other duties as deemed necessary by the Program Director and/or funding source DHS., • The Supervisor of Social Services will serve as the backup for the Director of Social Services. Scope of Position: Under the direction of the Program Director, and within the guidelines of all applicable laws, rules, regulations, and Agency code of ethics and policies and procedures, the Certified Peer Specialist supports adults with substance use disorders and/or mental health diagnoses. The Certified Peer Specialist respects the clients’ rights, maintains established professional boundaries, and delivers clinical services with the goal of assisting clients to achieve their personalized recovery goals. CRPA conducts outreach to both current program participants and within the communities of the Upper Manhattan and South Bronx conducting Narcan and Fentanyl test strip training to those who are at high risk of overdose and who have a friend/family/client who are at high risk of overdose. Essential Position Functions: • The Certified Peer Specialist engages with NYC homeless individuals beginning either at known “hang-outs” or during an inpatient hospital admission or emergency department visit with involvement in transition planning, including a needs assessment of community transition supports essential to stabilizing the participant., • Certified Peer Specialists facilitate Individual sessions with participants, emphasizing prevention and preparing for independent community living and the promotion of optimum mental and emotional health. May require helping participants deal with issues associated with but not limited to family and social relationships; stress and symptom management; activities of daily living; medication management; and housing readiness., • Certified Peer Specialists work with participants and their housing providers to resolve clinical issues that are impacting the participant’s ability to obtain, manage, and retain supportive housing., • Foster relationships with the community to ensure that recipients are connected with appropriate services as they transition back into the community., • Appointment navigation including accompaniment to appointments, travel training, re-engagement in community care, and addressing barriers to care., • Review documentation and conduct comprehensive psychosocial assessments to determine the medical, psychiatric, housing, and other social needs in the community., • Obtain historical and collateral information from multiple sources to support participants' behavioral and physical health needs., • Monitor, evaluate and record the participants progress with respect to care plan goals., • Attend and participate in team meetings and supervisory sessions., • Perform other related duties as assigned.Requirements:, • Bachelor’s Degree; Minimum three years of experience providing services to adults with serious mental illness preferred; Case Management work experience in a social service agency, preferably serving a behavioral., • Health population; Four years of past work case management work experience may be considered in lieu of a Bachelor’s degree., • Experience working with homeless and/or precariously housed populations preferred but not required; Knowledge of homeless resources, NYC shelter systems, and MTA transit systems a plus., • Knowledge of counseling principles and methods for mental illness and substance use disorders; Knowledge of treatment, rehabilitation, and community support programs as they relate to recipient/residents, families, and staff., • Ability to develop, evaluate, implement, and modify treatment intervention to meet the needs of individual recipients; ability to prepare accurate and timely reports; computer proficiency and good documentation skills; must have or be eligible to have NY State ID (non-drivers or drivers license)., • Knowledge and experience of electronic health records (specifically AWARDS by Footholds) preferred.

Job Description High School Counselor Classification Exempt, 10-Month Employee Reports to Director of Counseling Overview Dr. Richard Izquierdo Health and Science Charter School is seeking a certified/licensed School Counselor to support and educate our high school students. Under the general supervision of the Director of School Counseling, the school counselor works with students to develop and implement plans around academic and social-emotional success. School counselors work with the school community, including scholars, families, staff, and school leadership to advocate for programs and services that positively impact scholar academic success in alignment with New York State. The counselor is responsible for one grade level of students and is part of a robust mental health and counseling team at the school.
