The program is designed to provide long-term residential care and twenty-four-hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well-being. This 255-unit residence will provide housing for 153 individuals and families referred by the New York City HIV/AIDS Services Administration (HASA) and the rest 102 units will be apartments for moderate-income households. The Case Manager is responsible for providing supportive case management services to the residents referred by HASA. The successful candidate will have strong clinical and communication skills along with experience working with the HIV/AIDS population. Essential Job Tasks: Assists in Providing Clinical Services • Assigns and coordinates staff activities, • Provides clinical oversight, • Ensures staff operate according to all state and federal regulations and ICL policies and procedures, • Responsible for ensuring accuracy of and timely completion of required clinical documentation, • Responsible for ensuring service plans and goals are implemented as written and overseeing revisions to plans and goals as necessary, • Ensures appropriate crisis intervention occurs as needed, • Ensures staff receive relevant information regarding clinical treatment, program operations, community providers and resources, • Provides direct care coverage as needed, • Serves as Manager on Duty as assigned, • With the Program Director’s authority establishes and maintains work schedules in accordance with ICL policies and procedures, • Ensures assigned staff’s compliance with agency time and attendance policy, • Conducts staff supervision in accordance with division expectations and ICL procedures, • With the Program Director’s approval conducts disciplinary action as needed, • Assists in ensuring program is in compliance with all state and federal regulations and all ICL policies and procedures, • Ensures that Plans of Corrective Action are implemented and adhered to, • Regularly reviews and monitors staff activities and makes recommendations to Program Director, • Assists in the promotion of Mental Health and Wellness management in local communities, acts as liaison to community providers, community boards, elected officials, neighbors and emergency services providers, • Educates staff on the importance of maintaining positive relations with all factions of the community, • Cooperates with other organizations in the development of long range plans for mental health program development and wellness management., • Bachelor's degree in a recognized mental health discipline such as social work, psychology, nursing, rehab counseling, etc. plus a minimum of 2-3 years’ experience with direct supervision or program oversight with individuals with mental disabilities., • Ability to coordinate activities of staff and integrate as part of a multidisciplinary team, • Ability to manage multiple projects, delegate tasks and ensure completion of all tasks, assigned and delegated, • Ability to manage resources, • Ability to solve problems effectively, • Ability to work with consumers/residents, families, and staff in a caring and respectful manner, and with due understanding of and consideration for cultural differences., • Ability to serve as a role model to residents/consumers., • Ability to develop, evaluate, implement and modify a case management plan, meeting all deadlines and productivity standards, • Ability to complete written forms and reports in an accurate and timely manner., • Ability to communicate effectively with staff, consumers/residents, families, and the public., • Ability to prepare accurate and timely documentation, reports and other written material as assigned., • Ability to secure the cooperation of and work effectively with others, • Ability to work independently, and to conform to all applicable safety and accountability measures, • Ability to be aware of other’s reactions and understanding why they react the way they do, • Ability to actively looking for ways to help people, be compassionate and hopeful, • Ability to talk to others to effectively convey information, • Ability to listen to what other people are saying and asking questions as appropriate, • Ability to identify the nature of problems and propose solutions, • Ability to report for work as scheduled on a consistent basis, • Ability to understand and adhere to internal and external laws, rules, and policies, • Social Perceptiveness and trauma-informed perspective-ability to be aware of other’s reactions and understanding why they react the way they do

Job Description TITLE: Administrative Assistant/Entitlements Specialist REPORTS: Clinical Supervisor SALARY: $40,000 - $50,000 LOCATION: Tinton - 880 Tinton Ave. Bronx, NY 10473 Our Mission: Since 1967, Odyssey House has gone beyond helping individuals beat drugs and alcohol. MAJOR FUNCTIONS: Establish a liaison with social service agencies to expedite and ensure that residents receive appropriate benefits and that all funding guidelines, policies and procedures are followed. SPECIFIC DUTIES & RESPONSIBILITIES: • Receive resident applications, schedule interviews, provide admission response and coordinate all admissions., • Interview new inductees for SSI/SSD eligibility; fill out applications/requests for benefits; if needed obtain interim PA/Medicaid., • Prepare daily census, weekly/monthly reports; forward to appropriate government offices., • Maintain tax credit files on tenants., • Coordinate and ensure appropriate escort/advocacy for benefit court hearings for residents and other related hearings, meetings, etc., • Coordinate medical documentation of resident impairment., • Submit copies of Medicaid cards to collaterals, clinics, hospitals, etc. as needed., • Maintain files on resident identification and other pertinent materials affecting entitlement., • Submit billing on resident’s fees to appropriate program participants., • Perform as bursar and distribute monthly resident PNA., • Maintain and update accounts on all active cases., • Educate residents regarding entitlements. Work closely with resident’s case manager to facilitate successful discharge planning., • Coordinate referral for all entitlements at time of discharge (i.e., SSI/SSD, Medicaid, Food Stamps) to ensure smooth transition to new address or program., • Organize and maintain all tax credit documentation., • Maintain minutes for staff meetings, case conferences, community meetings, etc., • Track and distribute employee paychecks., • Attend regular scheduled staff meetings., • Attend all required in-service training seminars., • Meet monthly for supervision with Clinical Supervisor, • Bachelor’s Degree or High School/G.E.D with two (2) years experience in the social service field., • Must have some administrative, clerical and accounting experience., • Must be knowledgeable in the area of third-party reimbursement systems (SSI, PA, Food Stamps and other insurance and benefits)., • Must be computer literate including knowledge of Microsoft Word and Excel., • Must demonstrate excellent written, verbal, interpersonal and organizational skills.
