Job Description Astor’s Prevention Services/Family Connections is a time-limited, evidence-based model that utilizes a home-based approach to address both the concrete and clinical needs of the family. Prevention services utilize comprehensive family assessment tools to identify behaviors related to risk and to develop Core Outcomes that address the identified child welfare concerns within a family. The Case Planner provides in-home counseling services to families in need of support in developing skill and competencies that will enable them to keep their children in the home and out of placement. The Case Planner will maintain a caseload of 12-14 families and provide change focused interventions with families to improve parental functioning, address life stressors, and improve overall familial functioning. This is happen over a course of 6 to 12 month length of service and a minimum of 2 face to face contacts with each family per month. Job Description The Criminal Court program provides a safe reception center and various services to assist survivors and victims of crime navigate through the Criminal Court. The program provides counseling, crisis intervention, case management services, advocacy, information and referrals and practical assistance to survivors and victims of crime with an overall goal of promoting survivor safety and reducing the impact of victimization. The Coordinator is responsible for providing holistic support services to victims of all crimes. This included counseling and crisis intervention to assess the needs of victims and to help with the development of safety plans to address immediate risks and concerns. This plan may include the provision of in-person or telephone counseling; emergency assistance such as money for transportation; assistance with an Office of Victim Services (OVS) application; providing information and referrals, and follow-up services; and providing assistance with obtaining an order of protection, restitution, and other criminal justice remedies. The Coordinator will advocate on the client’s behalf with public entitlement programs, schools, employers, civil legal attorneys, and other community-based organizations, and coordinate and collaborate with the OVS Vocational Rehabilitation Unit and the New York Civil Legal Network to ensure that victims are receiving a comprehensive response. Job Description For over 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens and currently offers 160-plus programs and services for children, youth, adults, seniors, and those struggling with mental illness. Through the Benson Ridge and the Southwest Queens Senior Case Management Programs, case management and the homecare services are offered to the homebound elderly. The services include offering care management plans, assessments for the homecare, connections with the Home Delivered Meals and Friendly Visiting. The goal of the Case Management Program is to provide vital support services to keep frail seniors living safely in their homes and communities. STATEMENT OF THE JOB The Social Worker II is responsible for providing case management services to frail, homebound older adults, who have become unable to maintain daily living activities, making it possible for them to remain in their own homes.
