Job Description SUMMARY: The Primary Care Nurse Practitioner is responsible for providing comprehensive primary healthcare services to incarcerated individuals within an immigration detention facility and with a collaborating physician. This includes diagnosing and treating acute and chronic illnesses, managing preventive care, and coordinating with a multidisciplinary team to ensure the delivery of quality healthcare services within a medical home model. Location: 22844 Virginia Blvd, California City, CA 93505, United States Hours: Full-time, 40 hours a week Comp: $170,000 - $180,000 annually based on experience Benefits: • Health, • Dental, • Vision, • Life, • 401k w/ 5% match, • PTO ESSENTIAL FUNCTIONS: The successful applicant should be able to perform all of the following functions at a pace and level of performance consistent with the actual job performance requirements: Primary Healthcare Services: • Conduct thorough medical evaluations, physical examinations, and medical histories for incarcerated individuals., • Diagnose and treat a wide range of acute and chronic illnesses, injuries, and infections., • Develop and implement individualized treatment plans, prescribing appropriate medications, therapies, and interventions., • Order and interpret diagnostic tests, such as pharmaceuticals, laboratory work, radiological examinations, and screenings., • Provide vaccinations, perform health screenings, and promote preventive care measures within the correctional population., • Monitor and manage chronic diseases prevalent among incarcerated individuals, such as diabetes, hypertension, asthma, HIV/AIDS, and Hepatitis C., • Ensure proper medication management, including prescribing, adjusting dosages, and monitoring treatment effectiveness., • Educate patients on disease management strategies, lifestyle modifications, and medication adherence., • Respond to medical emergencies within the correctional facility, providing immediate assessment, stabilization, and necessary interventions., • Coordinate with emergency medical services or arrange for transfers to external medical facilities, when required., • Work collaboratively with a multidisciplinary team, including nurses, psychologists, psychiatrists, dentists, social workers, and other correctional staff, to ensure coordinated and integrated healthcare delivery., • Communicate effectively with healthcare team members, providing necessary information, consultations, and referrals., • Maintain accurate and up-to-date medical records, including patient histories, diagnoses, treatments, and medications administered., • Adhere to patient confidentiality regulations and ensure the security and privacy of medical records., • Stay updated with the latest advancements in primary care, correctional medicine, and relevant healthcare regulations., • Engage in continuous professional development activities., • Provide training and education to correctional staff on primary healthcare topics, as needed., • Excellent communication, interpersonal, and collaboration skills., • Ability to work effectively in a challenging and dynamic environment. QUALIFICATIONS: • Degree from an accredited Nurse Practitioner program., • Current and unrestricted Nurse Practitioner license in the state/jurisdiction where the correctional facility is located., • Experience in providing primary care or relevant experience in correctional or underserved populations is preferred., • Strong assessment and diagnostic skills, with the ability to manage a wide range of, • Excellent communication, interpersonal, and collaboration skills., • Ability to work effectively in a challenging and dynamic environment.
