Department Overview Intensive Mobile Treatment (IMT) is an innovative, creative, trauma-informed mobile practice model designed to provide treatment, rehabilitation and support services to individuals whose needs have not been met by traditional services. IMT participants interact with homeless services, criminal justice, and behavioral health service systems. IMT teams are hired, trained and funded to maximize flexibility and continuity of care for persons who historically have been poorly served. Community Access’ IMT Teams serve individuals who are street homeless or living between institutional settings and housing. Team members work collaboratively with settings in which the participant is living and use assertive engagement strategies to proactively engage individuals in services. IMT team members share responsibility for the people they serve, helping individuals attain housing and other personal and safety goals that may be of a health, social, work or education nature. IMT teams are multi-disciplinary and include psychiatry, nursing, social work, peer specialists and other staff members offering rehabilitation, treatment and recovery support services. The team provides expertise in the areas of housing, substance use and harm reduction, criminal justice, peer support and behavioral health recovery. Based on their area of expertise, team members collaborate to deliver comprehensive, integrated and flexible services that are responsive to a participant’s choices, needs and goals. Overview The IMT Peer Specialist Intern is an integral part of a multi-disciplinary team of mental health professionals. The Peer Specialist Intern is a resource to participants and other team members in issues related to integrating wellness goals. The Peer Specialist Intern is a person with the lived experience of a mental health condition who has a willingness to share personal and practical experience, knowledge, and first-hand insight to benefit IMT program participants. The Peer Specialist Intern engages and builds trusting relationships with program participants and their networks to support the person’s recovery. The IMT Peer Specialist Internship will provide you with the opportunity to develop the skills required to obtain a permanent position as a Peer Specialist within the workforce. Based on this information, completes and maintains consumer treatment plans to include quarterly service-plan reviews, assessments, medical records, and changes in treatment and/or medications either manually or electronically as instructed, • Provides supportive counseling in 1:1 or group formats that assist the consumer to modify attitudes and behaviors as needed., • Maintains up-to-date, accurate individual case records on each assigned consumer and develops measurable and objective service plans that maximize consumer rehabilitative abilities in accordance with regulatory guidelines. Continually documents consumer treatment progress. Reviews service plans and performs follow-up to determine quantity and quality of services provided. Ensures that plans include consideration of the consumer’s cultural and ethnic background, customs, needs, beliefs, and primary language., • Facilitates individualized services to the consumer that meet the diverse needs of the consumer and focus on the discharge-planning goal. Document case notes regarding significant and untoward events and contacts with mental health providers, including ICM’s and AOT’s., • Models’ appropriate behavior and provide rehabilitation and recovery services to assigned consumers that incorporates skills training in all areas of psychiatric rehabilitation, including the 11 restorative services. Advocates on behalf of consumer with outside service providers and within ICL and works with other team members in addressing the needs of the consumers., • Provides substance abuse services and administer urinalysis and/or breathalyzer tests to consumers as directed., • Performs crisis assessment and nonverbal and verbal crisis intervention using appropriate and approved techniques., • Participates in fire drills, performs headcounts, fire-watch and heat safety activities with other team members., • Immediately reports serious incidents, serious incident allegations, or sensitive situations to supervisors. Completes incident reports in accordance with ICL policy. Accounts for consumers and files missing person reports on consumers not accounted for in accordance with ICL policy and procedure., • Performs room checks and regular inspections of the consumer's rooms, using checklists or other aids, to ensure the safety of the consumers, accountability of ICL property, and cleanliness of consumer's residence. Engages consumer in learning how to maintain a safe, clean, and healthy personal living environment. Functions as liaison between consumer and facilities management department as needed., • Participates as instructed in all program communication and training activities: for example, shift briefings, daily floor meetings, communication log review and entry, supervision meetings, case conferences, staff meetings, and in-service training and development events. Responsibilities Summary Under close supervision, this position provides support and structure to residents in developing program plans by managing a caseload of client cases and administering the policies and procedures of the program as directed by the contracting agency and the Facility Director. Learns, uses and models the principles of the “What Works” system through evidence-based practices in all interactions with clients and other staff members. Trains and assists less experienced case managers. Primary Duties and Responsibilities • May perform the duties of a Monitor including resident intakes, check-ins, breath analysis and drug screens, to provide backup when necessary., • In own handwriting, prepares written personalized program for each resident to establish measurable criteria of expected behavior and accomplishments and a time frame for achieving specified goals., • Reviews plan with residents on a weekly basis and document progress and/or revisions, so resident is gaining the life skills necessary to make a successful re-entry into the community., • Assists residents in establishing personal budget, locating suitable housing, enrolling in educational and vocational programs, and participating in family and community activities, as appropriate, to ensure that they are working towards financial independence., • Monitors facility sanitation and safety. Assigns daily housekeeping chores to residents to comply with facility standards., • May orient residents upon arrival at facility to provide smooth assimilation into program (e.g. have resident complete paperwork, take on facility tour, introduce to other residents, etc.)., • Makes entries in own handwriting into confidential client (resident) files to provide the structure for case analysis and program planning and to provide documentation of program participation. Ensures files are secure to protect client confidentiality., • Assists Employment Placement Specialist to verify residents' jobs to ensure residents' employment is suitable under established program plan. Qualifications Minimum Requirements • Bachelor’s Degree in social, human services, criminal justice or behavioral science programs from an accredited college or university., • One (1) year of experience in human services, corrections or community-based services., • Work experience may be substituted for academic studies exchanging one (1) year of work experience in a related field for one (1) year of academic education., • Total work experience needed in lieu of the combination of education and work experience is five (5) years., • Familiar with community corrections systems and current community corrections practices., • Familiar with community work force needs, public and private agencies and employment services., • Ability to write clear concise reports in English using correct grammar., • Ability to add, subtract, multiply, divide, and calculate percentages., • Must be able to react quickly and appropriately in an emergency situation., • Bilingual in English/Spanish preferred, but not mandatory.
