SCHEDULE Monday – Friday, 8:00AM - 4:00PM SUMMARY Family Service League is seeking a full-time Case Manager for a homeless shelter in Brentwood, NY. The Case Manager will be responsible for conducting client assessments, developing Independent Living Plans, linking clients with community-based service resources. The Case Manager will assist clients in achieving self-sufficiency through advocacy, counseling, and resource referrals. $3,000 Sign-On Bonus! RESPONSIBILITIES • The Case Manager will complete new client intakes, explain shelter rules and regulations to clients., • Meet with clients at a minimum of twice per week for Face-to-Face meetings., • Develop Independent Living Plan for clients., • Complete Independent Living Plan bi-weekly., • Complete Mental and/or Physical Impairment form with Independent Living Plan Violations., • Complete SPA applications for eligible clients., • Work collaboratively with Housing Specialists in finding permanent housing options for clients., • Conduct twice weekly client safety inspections., • Address any issues of safety in unit, remove any dangerous items., • Address unit cleanliness and client hygiene. Assist client in daily living skills development., • The Case Manager will collaborate with the Office Manager on client shelter obligations., • Work with clients in creating a budget to identify needs and obligations. Where applicable, incorporate upcoming shelter obligation., • Review, complete follow up and submit FSL Client Incident Report to the Compliance Supervisor., • Maintain and update resource database of social service organizations relevant to the needs of client., • Provide ongoing support and counseling for clients in reaching their goals., • Provide crisis intervention to shelter clients., • Foster inter-agency collaboration to assist in clients’ long-term stability., • Maintain up-to-date documentation regarding client services in the Homeless Management Information System (HMIS) and client files., • Complete monthly reports in a timely manner. Submit documentation to DSS Housing Liaison., • Attend scheduled Case Management team meetings and individual supervision., • Enforce rules, regulations and policies of shelter., • Set and observe appropriate boundaries with clients; observe client confidentiality and HIPPA protocols., • Provides assessment, crisis intervention, individual, family, and group services to children, youth, and families., • Advocate for client needs., • Provide transportation as needed., • Work collaboratively with members of a larger treatment or service coordination team, both within the agency and in the larger community., • Model professional behavior., • Attend staff meetings and trainings as required., • Collaborate with the Security to discuss and resolve client incidents., • Each employee is responsible to read and understand the Policy and Procedure Manual. QUALIFICATIONS Bachelor’s degree in Human Services required. Master’s degree in Social Work preferred. Minimum of 1 year of experience in human services or related field preferred. Knowledge of the Suffolk County Department of Social Services preferred. Knowledge of human services and previous experience working with at-risk population preferred. Excellent interpersonal, verbal, and written communication skills required. Proficient computer skills, including Microsoft Office required; experience with HMIS, Google Apps, and web-based project management software preferred. Bilingual in Spanish preferred. Valid and clean NYS Driver's License required. PHYSICAL REQUIREMENTS Sitting and standing for extended periods of time, as well as moderate lifting. Must be able to go up and down stairs.

Job Description Position: Bilingual Case Manager| Full-time | Benefits Eligible | 35 hours per week FLSA status: Non-Exempt Hours: M-F, Days Department/Location: Fatherhood/Hauppauge PURPOSE The Long Island Fatherhood Initiative (L.I.F.I) program will address the needs of fathers 18 years & older (most low-income, many with history and involvement in the criminal justice system) in both Nassau & Suffolk Counties. The case manager's primary goal is to work with fathers by providing regular contact to facilitate program retention, completion, and access to needed support services with greater focus on economic stability and job-focused services with a clear and unwavering commitment to promoting responsible parenting and healthy marriage/relationship. SPECIFIC RESPONSIBILITIES 1. To provide systematic, coordinated outreach, intake, and comprehensive personal assessment services for all eligible participants referred by community-based agencies or identified through extensive outreach efforts, 2. Provide case management services to participants via phone, in person and/or virtually. Participants must receive a minimum of 8 case management service contacts, 3. Develop a targeted individualized case management plan with clients to identify their short- and long-term goals and assist them with their immediate including coordination of services and support as needed., 4. Provide direct services, including support, education, and focused external referrals to other needed services such as housing, substance abuse treatment, mental health programs, etc, 5. Convene weekly with case management team to review caseload, client progress, identify any barriers that could impact program retention/completion or any other programmatic challenges and successes., 6. Intensive focus on economic stability by coordinating needed economic stability supports, identifying obstacles and providing hand-on support to improve their readiness for job seeking and employment including follow-up on participants' progress regarding job placement, retention, and/or training., 7. Work as a part of a team to deliver a range of community outreach and engagement activities to promote services offered through the LIFI program to prospective partner organizations including but not limited to probation/parole, Drug and Family Court, CPS, homeless shelters, transitional and recovery housing facilities, 8. Methodically track and maintain records of all client contacts, goal objectives, progress and milestones including employment and education plans in nFORM. Ensure all entries are accurate, detailed and entered in a timely manner., 9. Participate in all professional development opportunities including but not limited to on-going training at conferences and webinars addressing Fatherhood, Healthy Relationships, Domestic Violence and Economic Stability issues., 10. Assist with the facilitation with nFORM surveys if needed, 11. Prepare reports as requested and other project duties as assigned including hotline coverage ORGANIZATIONAL RELATIONSHIPS ● Reports to: Program Coordinator ● Supervises: none QUALIFICATIONS • Bi-lingual (Spanish/English), • Bachelor's degree preferred with a minimum of 3 years-experience in the healthcare/social service field, • Lived experience with the family or criminal court system, whether personal or through a family member, is a plus and highly valued, • Good oral and written communication skills.
