Responsibilities: • Provide comprehensive supportive services to referred individuals by assessing their needs and situations, offering emotional and practical support, and coordinating services through collaboration with internal programs and external resources including facilitating connection to faith-based resources., • Incorporate Catholic Social teaching and values (such as compassion, integrity and service) into daily work and decision-making processes as appropriate, • Partner with Parish Ministers providing spiritual support while respecting clients personal beliefs., • Complete functional assessments, psychosocial reports and progress notes., • Facilitate communication and coordination between participants, families and service providers., • Meet the participant at their living space or in the community. Assist with scheduling appointments, follow-ups, and service visits as well as obtaining and maintaining benefits (Medicaid, Social Security, SNAP), • Consult/meet with various professionals, agencies, organizations regarding participants needs., • Track and update participant documentation., • Attend various staff meetings, education meetings/seminars to receive or provide information to other staff members and participant Qualifications: • Masters degree Social Work, Mental Health Counseling or Marriage and Family Therapy; LMSW, LMHC or LMFT preferred., • At least 1-year prior experience in related program and/or field work in graduate school preferred. Experience working with participants in various and sometimes intense situations., • Possess a basic knowledge of the Catholic Churchs doctrines and traditions (e.g. Catholic Social Teaching, Scripture, the Catechism), • Demonstrate a commitment to the values, beliefs, traditions, and moral teachings of Catholic Social Justice, such as compassion, integrity and service, • Bilingual capability (Spanish/English) preferred. Computer literacy required. Schedule: Full time 35 hours per week. Rate of Pay: $65,000 annual gross non-bilingual; $65,000 - $75,000 annual gross bilingual. This position is grant funded from 1/1/2026 through 12/31/26. This rate of pay serves as a good faith estimate. Responsibilities: Drive senior citizens and/or volunteers to and from their homes or apartments to the senior community service center or other places, ensuring safety procedures are followed. Ability to assist frail or handicapped individuals by lifting or pushing wheel chairs. Maintain accurate paperwork including log sheets of daily pick-ups, mileage, number of trips, fuel and oil receipts, money received. Assist with food service (meals) at centers. This includes breaking down and setting up dining tables, assisting with kitchen duties such as lifting heavy pots of hot water, taking trays of food in and out of oven for distribution. Assist center staff in setting up for and participating in recreational and/or social events as needed. Pick up and/or deliver various materials and supplies which may include trips to food stores, pharmacies, Hicksville Administrative office, etc. Qualifications: • HS Diploma/GED., • At least 2 years driving experience including knowledge of basic vehicle maintenance., • Current NYS valid CDL license with P Passenger endorsement., • English language capability., • A sincere desire to provide compassionate service to seniors., • Ability to lift/push to assist wheelchair-bound clients in boarding AND exiting bus., • Ability to assist frail clients in boarding and exiting bus., • Willingness to perform unrelated tasks at the center as needed. Responsibilities: • Maintain the overall condition, safety, and appearance of agency facilities and grounds through routine maintenance, repairs, and general services., • Perform basic household electrical repairs, installations, and appliance hookups and recognize when a licensed electrician is required by notifying the supervisor., • Implement routine household plumbing, maintenance, repairs and installations., • Complete routine carpentry, construction, alteration, repair and maintenance of buildings., • Clean and prepare surfaces for painting. Paint interior and exterior walls, ceilings, and cabinet work., • Maintain facility grounds, remove snow from walkways as directed, and perform general services., • Keep records, complete required paperwork, attend required meetings, and perform related duties as assigned.
