Role Description The Libertas Center for Human Rights at Elmhurst Hospital in Queens, NY is seeking a Social Worker to support the implementation of a new grant, focused on specialized eligible immigrant populations including refugees and asylees. The overarching project goal is to sustainably expand access to comprehensive, culturally responsive health and wellness services, through the delivery of health education, case management, and wellness groups that promote physical, mental, and social well-being. PURPOSE: The Kaplan House (Manhattan) provides therapeutic and support services to adolescent’s ages 17-21 who are referred to us through ACS and the Juvenile Justice system. Our residents have experienced significant trauma, resulting in behavioral, emotional, and functional/ development issues. Kaplan House offers an extensive independent living skills program, including vocational preparation, and educational services that assists youth towards reaching independence. POSITION OVERVIEW: The Clinical Director has full responsibility for the ongoing clinical and administrative guidance of the clinical staff. She/He also provided ongoing training, supervision and guidance to the clinical staff. The Clinical Director is the ITM (Integrated Treatment Model) liaison for the program. The Clinical Director is fully familiar with the Regulatory requirements and Agency/ Divisional clinical standards and practice. S/he functions as an internal resource on such matters. The Clinical Director is a senior member of the residential management team. In addition, s/he must provide safe, high quality, programs and adherence to the Jewish Board policies and procedures. The Organization integrates transitional housing with mental health services delivered through the principles of the Health Realization Approach. The Licensed Clinical Social Worker (LCSW) Supervisor is a part-time (0.4 FTE) licensed clinical position responsible for providing clinical supervision and oversight of the Comprehensive Care Coordination (CCC) services at the Freeman Residence Shelter, and clinical oversight of the Director of Social Services. This role delivers direct clinical services (small caseload), provides clinical supervision to clinicians, and supports the Program Director in all administrative aspects of clinic operations.
