The Center for Recovery and Wellness takes a holistic, strengths-based approach to recovery and wellness that responds to the distinct needs of each individual. Programs include residential and outpatient addiction treatment, recovery services, prevention education, and community programming. CRW offers culturally competent, evidence-based programs and services that strengthen individuals and communities, enhance resiliency, and foster lasting recovery. The Social Care Network Nutritional Counselor provides individualized dietary guidance and education to NYS Medicaid participants to promote optimal health and manage medical conditions through nutrition. This role involves assessing nutritional needs, developing personalized meal plans, and supporting clients in achieving their health goals. RESPONSIBILITIES • Conduct outreach to Medicaid participants who have been referred for nutritional services., • Conduct comprehensive nutritional assessments for clients based on medical history, lifestyle, and dietary habits., • Develop and implement personalized nutrition plans tailored to individual health needs and goals., • Educate participants on healthy eating habits, portion control, food labels, and nutrient-dense choices., • Monitor participant progress and adjust nutrition plans as needed., • Collaborate with healthcare providers, social workers, and wellness teams to support integrated care., • Maintain accurate and confidential participant records., • Provide group education sessions or workshops on nutrition-related topics., • Stay current with nutrition science, dietary guidelines, and public health trends., • Collaborate with care teams and community partners to ensure coordinated support., • Document interactions, referrals, and follow-ups accurately in case management platform, • Maintain confidentiality and act with cultural sensitivity and empathy., • Develop relationships with community groups/agencies in partnership with the Center., • Adhere to agency, SCN, NYS, and program-specific policies and procedures., • Attend staff meetings, required trainings, and other pertinent events as recommended by supervisors., • Bachelor’s degree in nutrition, Dietetics, Public Health, or a related field (Master’s preferred)., • Certification as a Registered Dietitian (RD) or Certified Nutrition Specialist (CNS)., • Minimum 2 years of experience in nutritional counseling or clinical nutrition., • Medicaid approved provider preferred, • Knowledge of chronic disease management (e.g., diabetes, hypertension, obesity), • Ability to motivate and support clients in behavior change., • Knowledge of social care systems, local resources, and health-related social need principles, • Familiarity with Medicaid eligibility, benefits, and referral systems, • Excellent communication, interpersonal, and active listening skills, • Technological proficiency with data entry, recordkeeping, and screening tools, • Some local travel is required for community site visits or events (monthly MetroCard / equivalent will be provided), • Must maintain professionalism in sensitive and confidential situations, • Flexibility as the agency rolls out enhanced care services The above job description is meant to describe the general nature and level of work being performed; it is not intended to be construed as an exhaustive list of all responsibilities, duties, and skills required for this position.
