CHI is looking for a full-time Traveling Case Manager for all of our Suffolk County locations. You will be responsible for ensuring that all program participants are provided with the services necessary to promote personal and economic independence, to have skills required to obtain, maintain, and retain permanent housing, and are living in well-maintained units. This is accomplished by performing the following functions: • Adheres to and enforces program regulations, policies, and procedures. Ensures that all clients are receiving quality services that are following the program requirements., • Assists clients with Shelter Managers in developing an Independent Living Plan (ILP) by identifying needs and goals and evaluates client’s progress on short and long term goals., • Provides information, referrals, counseling, crisis intervention, direct service, and advocacy services for all clients when directed., • Develops and maintains a resource network for referrals as related to the needs of the individuals being served in supervised programs. Collaborates with DSS, and other service providers to assist clients in fulfilling ILP and program requirements., • Assists clients with direct services such as current and projected client budget, current resume as appropriate, assistance with ADL skills, etc., • Responsible for face-to-face meetings with clients when manager is out., • Inspects living space regularly as required by the program to assess clients ADL needs and program compliance to assist clients with maintaining housing., • Limits and documents improper use of agency resources by clients such as unit damages, furniture, or appliances., • Evaluates, documents, and informs the direct supervisor of physical plant needs of the housing unit and/or common areas., • Completes discharge plans with clients., • Submits all internal and external reports in a timely and correct manner as required by funding agencies and/or CHI (logs, notes, incident reports). Maintains logs, office and site in an orderly fashion., • Must be able to travel to multiple sites in the Suffolk County region, • Submits all internal and external reports in a timely and correct manner as required by funding agencies and/or CHI (housing logs, unit inspection sheets, progress notes, ILPs, authorization forms required by DSS, special projects, etc.). Maintains charts and files in an orderly fashion., • Preps vacant units for new placements., • Maintains proper usage of cleaning supplies and inventory, • Facilitates and documents evacuation procedures, (i.e. fire drills) as directed by Program Manager, • At the direction of CHI staff (Program Manager or Director of Emergency Housing), acts as a liaison between the agency and the community at large., • Provide case management at all Suffolk County sites based on programmatic needs. WHAT DO I NEED? • Education: Bachelor’s Degree in human services or related field, Master’s Degree in Social Work preferred. Key Responsibilities Client Relationship & Facilities Program Management • Act as the primary liaison for assigned corporate clients, managing all aspects of their facilities programs across multiple sites., • Maintain overall accountability for the success and satisfaction of client accounts., • Build and maintain strong client relationships through proactive communication, regular performance reviews, and a consultative approach to solving facilities-related challenges., • Identify and support organic growth opportunities in service lines such as janitorial, construction, emergency response, and all services provided by W Services. Service Delivery Oversight • Monitor account performance and present strategic recommendations to clients., • Oversee the implementation of new services or site rollouts, coordinating with internal teams and vendors to ensure seamless transitions and consistent service delivery., • Monitor and close tickets within the Resolution Tracking System (RTS) while contributing to trend reporting and continuous improvement efforts., • Conduct audits and partner with field teams to ensure adherence to performance expectations and compliance requirements. Financial Accountability & Regulatory Communication • Support financial management of client accounts, including pricing proposals for new services, invoicing alignment, and account receivables follow-up when needed., • Communicate changes in labor regulations, minimum wage adjustments, and sales tax updates that may impact service pricing or delivery. Additional Responsibilities • Coordinate with internal departments—Field Operations, Procurement, Compliance, Construction, Finance, and Executive Leadership—to align client goals with service capabilities., • Participate in cross-functional meetings to identify areas of operational improvement and share client feedback for service enhancement., • When needed, provide after-hours support for critical client needs, urgent service requests, and escalations., • Special projects will be assigned as needed. About the Role The Coordinator of Legal and Compliance ensures the company adheres to compliance processes, regulatory requirements, and internal procedures. This role involves reviewing documentation, monitoring insurance compliance, supporting loss prevention and case management efforts, and handling high volumes of compliance inquiries. The coordinator also mentors staff, assists with risk mitigation, conducts audits, prepares reports, and maintains accurate records. Additionally, they educate employees on legal requirements and stay updated on regulatory changes to ensure ongoing compliance. KEY RESPONSIBILITIES: • Adheres to the company’s compliance processes and related workflows as implemented in the Standard Operating Procedure, • Follows procedures related to compliance, including but not limited to, assessment of onboarding documents, review of certificates of insurance and corresponding endorsements, IRS and banking documentation, ensures appropriate follow-ups are conducted, regularly communicates status to the appropriate parties, • Aids in the company’s Loss Prevention investigations processes, incident responses, and related workflows as documented in the department’s standard operating procedure, • Assist with Case Management projects as assigned, • Participates in training and mentoring of staff assigned to support in legal and compliance matters, • Responsible for follow up compliance replies, including assemble supporting documentation, deficiencies, and requests for information, • Ensure and monitor compliance for vendors with multistate insurance coverage, • Comfortable processing high volume of inbound and outbound compliance support calls with excellent phone etiquette, • Monitor existing contractor partner database to ensure contractors are compliant with W Services insurance/contract requirements, • Process cancelation notices that come in VIA mail immediately in company database and reach out to the contractor partner/ insurance company for immediately resolution to avoid a lapse in coverage, • Develop and maintain professional rapport with all necessary parties; being mindful of our relationships (i.e., corporate client, contractor, vendor, store manager, etc.), • Update all mandatory sections of the company database in a timely manner, • Responsible for escalating priority insurance concerns to senior manager and/or the appropriate unit, • Help implement policies, systems, or controls to reduce risk, • Use routine audits to identify potential compliance risks, • Prep reports and updates for weekly meeting with management, • Participate in monthly department meetings, • Monitor business processes, • Educate employees about regulatory and legal requirements, • Stay up to date with regulatory changes ADDITIONAL RESPONSIBILITIES • Experience with MS Word, Excel & Outlook, • Must be a team player and have a positive demeanor (enthusiastic), • Able to multi-task, • Must have excellent communication, organizational skills and decision-making skills, • Able to work under a demanding and busy work environment
